Borstkankerdiagnose dankzij kamelen

Wereldwijd zijn er jaarlijks naar schatting 1,4 miljoen vrouwen die te horen krijgen dat ze borstkanker hebben. Dit maakt van borstkanker met voorsprong de meest voorkomende vorm van kanker bij vrouwen. Desondanks vormt borstkanker niet de meest dodelijke vorm onder de kankers. Een belangrijke reden hiervoor is het toenemende gebruik en de grotere effectiviteit van de beschikbare behandelingen. Voor de bepaling van de meest geschikte behandeling vormt de diagnose van het type borstkanker een belangrijke stap. Kamelen lijken hiervoor een pijnloze oplossing te bieden.

Een belangrijke en agressieve vorm van borstkanker is de HER2-positieve borstkanker. HER2 (de humane epidermale groeifactorreceptor 2) is een eiwit dat abnormaal veel voorkomt op deze kankercellen. Er is momenteel reeds een behandeling beschikbaar die zich richt tegen deze specifieke vorm van borstkanker. Maar om in aanmerking te komen voor de behandeling is een correcte diagnose van het type borstkanker vereist. De huidige beschikbare technieken vereisen hiervoor een biopsie, waarbij er gesneden moet worden in de borst van de patiënt om een stukje van de tumor te onderzoeken, wat uiteraard een uiterst onaangename ervaring is. Medische beeldvorming vormt een niet-invasief alternatief hiervoor, dit wil zeggen dat er niet meer gesneden hoeft te worden. Door medische beeldvorming wil men specifiek de HER2 tumor in beeld brengen, net zoals men bijvoorbeeld in staat is via X-stralen het menselijke geraamte in beeld te brengen. Na de inspuiting van een beeldvormingsprobe gericht tegen het HER2 eiwit kunnen we de tumor lokaliseren en identificeren, en kunnen we zelfs uitzaaiingen opsporen. De beeldvormingsprobe is eigenlijk vergelijkbaar met een speurhond die we hebben opgeleid om een bepaald type kanker, in dit geval HER2-positieve borstkanker, op te sporen. Omdat we aan de beeldvormingsprobe ook een merker koppelen, zijn we in staat deze in beeld te brengen, net alsof we een lamp vastmaken aan onze speurhond waardoor deze zichtbaar wordt in het donker.

Momenteel wordt er onderzoek verricht om ook afgeleiden van kameelantilichamen als beeldvormingsprobe te gebruiken. Antilichamen vormen een belangrijk wapen in het verdedigingssysteem van mens en dier, en zijn dan ook in staat een vijand te herkennen en alarm te slaan. De kameelachtigen (kamelen, dromedarissen, lama’s,…) beschikken naast de conventionele antilichamen (die vergelijkbaar zijn met de antilichamen die voorkomen in het menselijke lichaam) ook over zogenaamde zware keten antilichamen. Dit type antilichamen biedt het voordeel dat er op eenvoudige wijze een klein fragment kan afgezonderd worden dat de functie van speurhond op zich kan nemen, iets wat bij de conventionele antilichamen niet zo vanzelfsprekend is. Omwille van hun uiterst kleine omvang (in de orde van enkele miljoensten van een millimeter) worden deze fragmenten ‘Nanobodies’ genoemd. Het gebruik van zulke kleine fragmenten is interessant omdat ze de tumor sneller en gemakkelijker kunnen bereiken, en bovendien zullen diegene die de kankercellen niet vinden ook weer sneller uit het lichaam van de patiënt verdwijnen. Dit voorkomt vals-positieve signalen en verhoogt ook het contrast van de beeldvorming, met name het onderscheid tussen tumorcellen en niet-tumorcellen.

Deze scriptie handelt meer bepaald over de wijze waarop een merker kan vastgemaakt worden aan een Nanobody. Momenteel gebeurt dit vaak door de merker te koppelen aan de lysines die voorkomen op het oppervlak van het Nanobody. Lysine is één van de twintig natuurlijk voorkomende aminozuren, de samenstellende delen van Nanobodies en andere eiwitten. Omdat lysine een veel voorkomend aminozuur is in Nanobodies, is het grote nadeel van deze koppelingsmethode dat dit op een willekeurige manier gebeurt: men weet op voorhand niet hoeveel merkers zullen binden en op welke plaatsen deze zullen binden. Net alsof er één, twee of drie lampen op verschillende plaatsen in de vacht van de speurhond vastgemaakt worden. Het grootste risico hierbij is dat het Nanobody zijn functie, namelijk het opsporen en binden van het doelwit, zou kunnen verliezen. Dit is het geval wanneer de merker bindt in de antigen-bindende regio van het Nanobody. Deze regio is een belangrijk onderdeel voor de functionaliteit van een Nanobody en zou dan ook best kunnen vergeleken worden met de snuit van de speurhond: wanneer we een lamp zouden vastmaken aan de snuit van de speurhond, zal dit zijn vermogen om zijn neus te gebruiken om de vijand op te sporen verminderen.

In deze scriptie wordt aangetoond dat het ook mogelijk is de merker op een site-specifieke manier te koppelen, waarbij we een positie kiezen waaraan één merker vastgemaakt wordt. Er wordt dan ook best gekozen om de merker aan het uiteinde tegenover de antigen-bindende site vast te maken, vergelijkbaar met de situatie waarbij de lamp zou vastgemaakt worden aan de staart van de speurhond, ver weg van de snuit. Zo kunnen we de hond volgen, zonder dat hij gehinderd wordt. Hiervoor wordt een cysteïne, een ander aminozuur, geïntroduceerd aan het uiteinde van het Nanobody. Er wordt aangetoond dat de beeldvormingsprobe na aanhechting van de merker nog steeds functioneel is en dus nog in staat is HER2-positieve tumoren te herkennen.

Het gebruik van deze site-specifieke koppelingsmethode resulteert in twee belangrijke voordelen. Door het specifiek kunnen kiezen van de positie waar de merker gekoppeld wordt, bestaat het risico niet meer dat de Nanobodies hun functie zouden verliezen en dus onbruikbaar zijn voor de diagnose in de patiënten. Een tweede voordeel is dat een groep beeldvormingsprobes ontwikkeld wordt waarvan elke probe slechts één merker heeft. Omdat we nu geen heterogene groep meer hebben (waarbij er probes zijn met één merker, probes met twee merkers,…) resulteert dit ook in een snellere overdracht van de beeldvormingsprobe van het onderzoekslabo naar het klinische gebruik in mensen, eenvoudigweg omdat er minder karakteriseringswerk vereist is.

Met dit onderzoek wordt dan ook een belangrijke stap genomen in de ontwikkeling van een pijnloze diagnosetechniek bij borstkankerpatiënten, met het oog op het kiezen van de meest geschikte behandeling.
