Therapietrouw: antiretrovirale therapie

 Helft Belgische hiv-patiënten niet therapietrouw

Een studente farmacie van de KULeuven vond dat ongeveer de helft van de Belgische hiv-patiënten zijn hiv-medicatie niet neemt zoals het hoort. De belangrijkste oorzaken hiervan zijn het taboe dat heerst rond de ziekte en de nevenwerkingen die de geneesmiddelen hebben.

Wereldwijd zijn er meer dan 33 miljoen mensen besmet met het hiv-virus. In België zijn er dat ongeveer 14000. Er is een verschil tussen hiv en aids. Hiv is het virus, dat kan worden doorgegeven door onbeschermd seksueel contact maar ook door rechtstreeks bloedcontact zoals via wonden. Ook een moeder kan het aan haar kind doorgeven bij diens geboorte. Aids is de ziekte die men kan krijgen wanneer het virus niet onderdrukt wordt. Dat onderdrukken gebeurt door medicatie speciaal gericht tegen het virus. De bestaande medicatie kan het virus echter niet doden waardoor de patiënt heel zijn leven de medicatie moet nemen. Overigens krijgt niet elke hiv-patiënt onmiddellijk medicatie: dit gebeurt pas op aanraden van en in samenspraak met de behandelende arts. Patiënten die wel geneesmiddelen nemen, moeten deze dag in dag uit en vaak meerdere keren per dag innemen. Dat vereist veel discipline. Een patiënt moet immers minstens 95% van zijn medicatie op de juiste manier en het juiste moment innemen opdat het virus er niet resistent aan zou worden. Resistentie betekent dat het virus niet meer onderdrukt kan worden door de medicatie en dit door veranderingen aan het virus zelf. Indien een patiënt 95% van diens medicatie op de juiste manier en het juiste moment inneemt, is hij of zij therapietrouw.

Wetenschappelijke artikels gaven al aan dat veel hiv-patiënten niet therapietrouw zijn. Maar hoe zit het precies met Belgische patiënten? Om dit te weten te komen, werd door een laatstejaarsstudente farmacie aan de KULeuven een project opgestart. In het eerste deel van dat project onderzocht ze een databank waarin de therapietrouw van ongeveer 160 Belgische hiv-patiënten berekend werd. Hieruit bleek dat de helft van de hiv-patiënten hun medicatie niet elke dag neemt en dus therapieontrouw is.

In het tweede deel onderzocht de studente de redenen voor het al dan niet therapietrouw zijn. Hiervoor interviewde ze 10 Belgische hiv-patiënten. In deze interviews werd gevraagd wat het hen moeilijk of makkelijk maakt hun medicatie elke dag in te nemen. Iedere patiënt heeft zo zijn eigen redenen om de medicatie al eens niet in te nemen. Zo is bijvoorbeeld het taboe rond de ziekte een belemmerende factor om geneesmiddelen in het openbaar te nemen. Ook de nevenwerkingen van hiv-medicatie zoals misselijkheid of slaapproblemen, kunnen ervoor zorgen dat de patiënten al eens een pilletje minder nemen. Veel patiënten vinden het ook moeilijk hun ‘pillenschema’ in te bouwen in hun dagelijkse leven en dit vooral tijdens de eerste maanden van hun therapie. Velen vergeten immers gemakkelijk hun geneesmiddelen in te nemen als ze met iets anders bezig zijn.

De patiënten gaven ook aan wat er hen zou kunnen helpen om therapietrouw te zijn of te worden. Uiteraard is steun uit hun omgeving en het doorbreken van het taboe rond de ziekte heel belangrijk. Sensoa® is hier al jaren mee bezig maar hopelijk kan ook HIV+, het tv-programma op Eén, hier verandering in brengen. Wat eveneens kan helpen is het hanteren van een alarm of zelfs het krijgen van een sms’je van de firma die de medicijnen maakt, waarin staat dat het tijd is om ze in te nemen. Ook het beter inbouwen van medicatie in het dagelijks leven kan therapietrouw bevorderen. Dit kan men doen door het aantal innamemomenten per dag te beperken. En ook het leggen van medicatie op verschillende plaatsen (in de keuken, in de slaapkamer, bij familie of vrienden) en het gebruik van een pillendoos kunnen volgens de patiënten voor een betere therapietrouw zorgen. Een pillendoos bevat 7 compartimenten, voor elke dag van de week één. Bij de meeste pillendozen bevat elk compartiment nog eens meerdere vakjes voor de verschillende innamemomenten op die dag. In die vakjes komt dan de medicatie en zo kan de patiënt makkelijk zien of hij al dan niet zijn medicatie genomen heeft en heeft hij ook altijd de medicatie bij. Veel patiënten willen ook voldoende informatie over de ziekte, over de werking van de medicijnen, wat ze ervan kunnen verwachten, enzovoort.

De conclusie van het onderzoek is dat niet elke patiënt baat heeft bij elke maatregel. Daarom zou het optimaal zijn dat hulpverleners zoals artsen, apothekers of verpleegkundigen een gesprek hebben met de patiënt en daarin polsen wat men voor hem of haar kan doen om zijn of haar therapietrouw te bevorderen.
