3D analyse van het kniegewricht. Anatomie en reconstructie

 

3D – analyse van de kruisband: nieuwe inzichten in anatomie en reconstructie.

Een gescheurde kruisband, blessure van de topsporter?

Van Nistelrooy, Xavi, Afellay, Ronaldo, Owen... dit is slechts een greep uit de klinkende namen van professionele voetballers die minstens éénmaal een kruisbandruptuur opliepen. Niet alleen voetballers worden geconfronteerd met dergelijke letsels, zo werden Tiger Woods, Mary Pierce en Derrick Rose ook slachtoffer van deze veel voorkomende knieblessure. Veel voorkomend? We hoeven het niet eens tot in het topsportmilieu te gaan zoeken; zo worden in de Verenigde Staten jaarlijks meer dan honderdduizend voorste kruisbandreconstructies uitgevoerd. Nu, waarom zijn deze ligamenten zo belangrijk voor de functie van het kniegewricht?

 

Impossible is nothing

Zowel de voorste als achterste kruisband spelen een uitermate belangrijke rol in het garanderen van de stabiliteit van de knie. Letsels ter hoogte van deze ligamenten kunnen leiden tot significante instabiliteit van de knie en bijkomende schade, zoals meniscusscheuren en beschadiging van het kraakbeen. Een gedetailleerde bespreking van de anatomische bouw en de technische kant van de reconstructieve technieken zou ons hier te ver leiden, maar het is belangrijk te weten dat zonder verregaande kennis van de anatomische bouw en biomechanica van deze structuren, onmogelijk een volledig en correct herstel kan beoogd worden.

De afgelopen tien jaar is er in de medische wereld daarom intensief onderzoek gevoerd naar de anatomie, biomechanica en functie van de humane kruisbanden. De techniek van voorste en achterste kruisbandreconstructie kende op basis van deze wetenschap een grote evolutie met ontwikkeling van verschillende reconstructieve technieken.  In lijn van het adagio ‘wetenschap staat nooit stil’ wordt koortsachtig verder gezocht naar nog meer verfijnde technieken met nog betere resultaten dan de vorige. Hierbij kunnen we ons echter de vraag stellen waarom innovatie van technieken, waarvan we uitgaan dat ze de standaard zijn, nodig is?

Niet alleen de anatomie en de reconstructie van de kruisband is aandacht van onderzoek, ook de resultaten van reconstructie, zowel op korte als lange termijn, zijn op heden een interessant en vooral belangrijk topic voor research. Verschillende recente studies toonden aan dat, ondanks de goede tot uitstekende resultaten op korte termijn, vaak progressie gezien wordt naar osteoartrose na verloop van tijd. Deze vaststelling heeft ertoe geleid op zoek te gaan naar nieuwe inzichten betreffende de anatomie en reconstructie van de kruisband.  

Kwaliteit is het uitsluiten van toeval

Gebaseerd op vernieuwende 3D CT beeldvorming, was het doel van deze studie vijfdelig. Het eerste deel bestond uit (1) visualisatie van de beenderige aanhechtingen van de kruisband ter hoogte van het boven- en onderbeen, (2) het in beeld brengen van het kruisbandweefsel met omliggende structuren en (3) het gedetailleerd visualiseren van het verloop van beide ligamenten. Het tweede luik van dit onderzoek bestond uit het analyseren en vergelijken van de ruimtelijke oriëntatie tussen enerzijds de anatomische positie en anderzijds de plaats van chirurgische reconstructie (4). Tenslotte werd een vergelijkende studie opgezet tussen twee courant gebruikte technieken voor kruisbandherstel (5).

 

Succes noodzaakt voorbereiding

Voor het bestuderen van de normale anatomie werden negen gebalsemde knieën gebruikt waarbij de kruisbanden na dissectie geïnjecteerd en gekleurd werden met een contrastmedium voor visualisatie op de Computed Tomography scanner (CT-scan). Deze beelden werden gesegmenteerd en gereconstrueerd in 3D, wat morfologische en morfometrische analyse toeliet van de voorste en achterste kruisband. Vervolgens werden de aanhechtingen van de kruisbanden aan het bot in beeld gebracht, werden cylinders gereconstrueerd doorheen de ligamenten en werd berekend welk oppervlak deze cylinders innamen na projectie op de insertie-oppervlakken. Verder werd de centrale as van iedere kruisband bepaald en werden de centra van iedere aanhechting georiënteerd en gelokaliseerd op het bot aan de hand van hiervoor ontworpen roosters; dit voor volledige anatomische beschrijving van de normale, humane kruisband.

Deze geometrische modellen werden gevalideerd en vergeleken met postoperatieve CT-beelden van patiënten die een kruisbandreconstructie ondergingen; hierbij werd verder een vergelijking gemaakt tussen de anteromediale en de transtibiale techniek. Bij de eerste techniek wordt de tunnel voor de fixatie van de nieuwe kruisband geboord via een extra opening in de knie; in tegenstelling tot de transtibiale techniek waarbij men de tunnel in het bovenbeen gaat boren via de tunnel in het onderbeen.

Just do it

Zoals de bijgevoegde afbeeldingen weergeven, biedt deze techniek de mogelijkheid om een zeer gedetailleerd beeld te krijgen van de anatomische bouw en oriëntatie van de kruisbanden. Verder laat het ons toe nauwkeurige metingen uit te voeren, wat van belang kan zijn bij preoperatieve planning van chirurgisch kruisbandherstel. Bij analyse van de resultaten uit deze studies werd besloten dat er een zeer grote variatie bestaat binnen de anatomische bouw van de kruisband, laat staan binnen het kniegewricht tout court. Een opmerkelijke conclusie was dat de aanhechting ter hoogte van het bovenbeen veel groter was dan de aanhechting op het onderbeen. Verder kon ook besloten worden dat de oriëntatie en spanning op de kruisbanden sterk verschilt bij plooien of strekken van de knie. Voor wat betreft het luik over de heelkundige technieken, kwamen we tot het besluit dat er bij de techniek met de extra insnede in de knie, opmerkelijk kortere tunnels en een kleinere hoek tussen de beide tunnels gemaakt wordt en deze techniek er in slaagt een meer anatomisch of een meer natuurgetrouw herstel te geven, wanneer we gaan vergelijken met de andere techniek.   

 

Je wint geen zilver; je verliest goud!

Als conclusie kunnen we stellen dat deze vernieuwende 3D CT studies buitengewone informatie en inzichten bieden over de normale voorste en achterste kruisband, dewelke geïmplementeerd kunnen worden in de chirurgische praktijk. Deze studies zijn dan ook de eerste die de gedetailleerde anatomie van de menselijke voorste en achterste kruisband gaan beschrijven in drie dimensies met respect voor het verloop en de aanhechtingen op het bot en ze geven nieuwe inzichten in de complexe materie van de orthopedische reconstructieve heelkunde. Dit geheel aan wetenschap kan een belangrijke rol spelen bij de verdere ontwikkelingen in de wereld van de orthopedische chirurgie, met het oog op verder optimaliseren van behandelingsstrategieën.
