Dit artikel beschrijft de taalvaardigheid van kinderen met rolandische epilepsie en de meest recente ontwikkelingen binnen de structurele beeldvorming. Op basis van Diffusion Tensor Imaging (DTI) wordt een poging gewaagd om een neuro-anatomisch substraat te vinden dat aan de basis ligt van de taalproblemen bij deze kinderen.

Inleiding
Benigne rolandische epilepsie is het meest voorkomende idiopathische epilepsietype bij kinderen. De epileptische episodes beginnen gewoonlijk tussen vier- en tienjarige leeftijd en verdwijnen in de puberteit, zowel met als zonder medicamenteuze behandeling[1],[2]. Aanvallen ontstaan focaal en tasten de hersenregio’s rond de rolandische fissuur aan.

Milde cognitieve problemen worden steeds vaker (h)erkend in het klinisch beeld van deze epilepsie[1]. Er zou een oorzaak-gevolg relatie zijn tussen het voorkomen van chronische epileptiforme ontladingen tussen de aanvallen door en cognitieve dysfuncties. Bijgevolg kunnen we de benigne aard van deze aandoening meer en meer in vraag stellen.

Verschillende onderzoeken rapporteren uiteenlopende taalbeperkingen bij kinderen met rolandische epilepsie. Moeilijkheden met fonologische- en literaire vaardigheden (lezen en spellen) worden het meest gerapporteerd[4],[5]. Ook worden beperkingen in de expressieve grammatica waargenomen. Riva en collega’s[6] vinden dat deze kinderen milde taalbeperkingen hebben op het vlak van fonemische vaardigheden, begrijpend lezen en semantische kennis. Verotti en zijn medewerkers[7] besluiten dat er problemen zijn met de receptieve en expressieve woordenschat.

We kunnen er dus van uit gaan dat deze kinderen minder taalvaardig zijn dan controlekinderen zonder epilepsie. Eerder dan een globaal verminderde taalvaardigheid, verwachten we een selectief profiel: kinderen met rolandische epilepsie scoren voor bepaalde taaldomeinen minder goed dan voor andere.

Waarschijnlijk verspreiden de epileptiforme ontladingen zich in de hersenen vanuit de rolandische fissuur naar de perisylvische cortex (waar belangrijke taalcentra gelokaliseerd worden) en oefenen ze zo een negatieve impact uit op de taalontwikkeling. Deze taalontwikkeling gaat vanzelfsprekend gepaard met hersenontwikkeling. DTI stelt ons in staat de zenuwbundels in de hersenen te visualiseren. Deze niet-invasieve beeldvormingstechniek maakt het mogelijk de beweging van watermoleculen langsheen zenuwvezels zichtbaar te maken en te kwantificeren middels een Fractionele Anisotropie (FA)-waarde; een getal tussen 0 en 1.

De voornaamste taalbaan in de hersenen is de Superior Longitudinale Fasciculus (SLF). Deze zenuwbundel verbindt de klassieke taalgebieden (het gebied van Broca en het gebied van Wernicke) met elkaar. De maturatie van deze witte stofbaan gaat door tot de leeftijd van 13 jaar. Structurele maturatie van zenuwvezels in het brein wordt gedeeltelijk gekarakteriseerd door een toenemende myelinisatie van deze vezels. Myeline is een vetachtige witte stof die rond de zenuwvezels wordt gevormd en een snelle signaaloverdracht over de zenuwbundels bewerkstelligt. DTI vertaalt dit in een toename van de FA-waarde[8]. Een hogere FA-waarde duidt op een hogere integriteit. FA-waarden verschaffen ons dus informatie over de integriteit van witte stof, myelinisatie, axondensiteit en vezeloriëntatie.

De hersenactiviteit die gepaard gaat met het verwerken van taal kent reeds een linkerhemisferische dominantie bij kinderen van drie jaar oud[9]. Voortdurende functionele- of traag progressieve structurele verstoringen als gevolg van de chronische aard van de epileptische activiteit zou kunnen resulteren in een verschuiving naar rechter hemisferische taaldominantie[10] of een routewijziging van taalbanen naar minder traditionele taalgebieden binnen de dominante linker hemisfeer[11],[12].

Methode en hypothesen
We verwachten dat kinderen met epilepsie een verminderde graad van myelinisatie vertonen van de SLF in vergelijking met controlesubjecten zonder taalproblemen. We trachten mogelijke verschillen vast te stellen tussen de FA-waarde van de SLF bij kinderen met rolandische epilepsie en zich normaal ontwikkelende controlekinderen. Daarnaast onderzoeken we of beide groepen verschillen met betrekking tot de lengte van deze taalbaan. We maken voor al deze structurele beeldvormingsmaten van de SLF een vergelijking tussen de linker- en rechter hemisfeer en trachten uitspraken te doen over taaldominantie. Met behulp van twee voorname gestandaardiseerde taaltests (Clinical Evaluation of Language Fundamentals 4 Nederlandstalige versie, Peabody Picture Vocabulary Test 3 Nederlandstalige versie) proberen we het talig profiel waarmee deze kinderen zich presenteren te achterhalen. We onderzoeken voor welke taalaspecten kinderen met rolandische epilepsie het significant minder goed doen dan gematchte controlekinderen zonder epilepsie. Tot slot proberen we de verschillende beeldvormingsmaten te correleren met de scores op de talige testing. Vijf kinderen met rolandische epilepsie en vijf gezonde controlekinderen worden gematcht volgens leeftijd en geslacht.

Resultaten
Kinderen met rolandische epilepsie blijken minder taalvaardig te zijn dan gezonde gematchte controlekinderen. Ze scoren significant lager op de Taalinhoud Index (TII) van de CELF-4NL en op enkele andere semantische subtests (Begrippen en Aanwijzingen Volgen, Woordcategorieën Expressief, Zinnen Formuleren en Semantische Relaties). Er wordt geen verschil tussen beide groepen gevonden voor de waarden van de verschillende structurele parameters van de SLF, doch wel worden significante correlaties weerhouden tussen de semantische subtests en deze beeldvormingsparameters bij kinderen met rolandische epilepsie. Deze correlaties kunnen we niet terugvinden bij gezonde controlekinderen. Daarnaast blijken deze kinderen een minder uitgesproken linker hemisferische dominantie voor taal te vertonen.

Conclusie
We kunnen besluiten dat kinderen met rolandische epilepsie in vergelijking met gezonde controlekinderen meer moeilijkheden hebben met de semantische verwerking van taal. We zien significante correlaties met structurele beeldvormingsmaten van de SLF. De literatuur kent inderdaad semantische functies toe aan de SLF[13]. Semantiek wordt gedefinieerd als de betekenis van taal en betekenisrelaties binnen taal. Dit omvat de betekenis van onder andere woorden, zinnen, verhalen en spreekwoorden[14]. Het semantisch systeem wordt door Ellis en Young gezien als het centrale systeem voor alle taalverwerking[15]. Het lijkt er op dat er een neuro-anatomisch substraat is dat uniek is voor het talig profiel dat we aantreffen bij kinderen met rolandische epilepsie.

Rolandische epilepsie werd gedurende een lange tijd gezien als een epileptisch syndroom met een zeer goede prognose, maar kan niet benigne worden genoemd in het licht van de impact op cognitieve vaardigheden. De epileptische activiteit die we aantreffen in de hersenen van kinderen met rolandische epilepsie zou de taalontwikkeling kunnen verstoren. We willen deze kinderen en hun taalontwikkeling beter kunnen begeleiden. Beeldvormend onderzoek van grotere groepen kan een bijdrage leveren met betrekking tot het vinden van een neuro-anatomisch substraat voor de taalproblemen bij kinderen met rolandische epilepsie en vroegdiagnostiek in de hand werken.
