The viability of the lipoaspirate after sieving into different particle size fractions, an experimental study

Plastisch chirurgen willen de herhaalde operaties na een borstreconstructie overbodig maken

In de Vrije Universiteit Brussel (VUB) en het verbonden Universitaire Ziekenhuis Brussel wordt onderzoek gevoerd om meer succes te boeken met lipofilling. Dit is een techniek waarbij vetweefsel door liposuctie wordt weggezogen en vervolgens opnieuw ingebracht. Uiteraard bij dezelfde patiënte, maar wel op een andere plaats, zoals bv. in een gereconstrueerde borst. Het resultaat van de operatie wordt bepaald door talrijke factoren en dit heeft ertoe geleid dat lipofilling vandaag een hot topic is in de research wereld. Ook in het UZ Brussel dragen de plastisch chirurgen hun steentje bij. Zij hebben een rapport uitgebracht van de eerste resultaten die ze geboekt hebben.

Vrouwen die een borstamputatie hebben ondergaan in de behandeling van borstkanker, hebben niet steeds dezelfde wens. Sommige patiëntes willen het volledig proces zo snel mogelijk beëindigen en blijven liever weg uit ziekenhuizen. Eventueel kunnen zij een uitwendige prothese dragen. Andere vrouwen opteren voor een operatie die hen een nieuwe borst schenkt. Dit kan met een inwendige prothese of met een zogenaamde flap die bestaat uit lichaamseigen weefsel. Dankzij de ontwikkelingen tijdens de laatste decennia in de plastische chirurgie, kiezen vrouwen steeds vaker voor deze laatste optie. Uiteraard wordt nooit een perfecte kopie gecreëerd en blijven correcties wenselijk.

Lipofilling

En die correcties kunnen bekomen worden met lipofilling. Terwijl het opgezogen vetweefsel bij liposuctie meestal wordt vernietigd, kan het bij deze patiëntes gebruikt worden als filler. Stap per stap kan het vetweefsel geïnjecteerd worden om de borst meer volume en een natuurlijke ronding te geven. Het weefsel is zacht en warm en het uiteindelijke doel is om weer symmetrie te krijgen tussen de linker- en rechterkant. Bovendien gaat het om het inspuiten van lichaamseigen weefsel wat het risico op allergische reacties of verwikkelingen na de operatie zeer klein maakt. Dit klinkt veelbelovend en toch is er een nadeel. Het resultaat gaat niet levenslang mee. Zelfs al na enkele weken, is er vermindering in volume zichtbaar. Soms blijft na een jaar slechts de helft van het ingespoten materiaal over en dringt een tweede, en eventueel een derde, lipofillingsessie zich op.

Hot topic

Dit heeft geleid tot een massa aan onderzoeksprojecten over de hele wereld. Vinden we beter vetweefsel in de buik of in de billen? Hoe lang steken we dit geaspireerde vetweefsel in de centrifuge om het vetweefsel te concentreren? Hoe snel laten we de centrifuge draaien? Op welke manier injecteren we het vet opnieuw in de patiënte? Elke stap in het proces van liposuctie tot lipofilling is ondertussen bestudeerd. De basis van het onderzoek is de cell survival theory, die ondertussen meer dan een halve eeuw oud is. Volgens deze theorie zal het resultaat langer behouden blijven als levende vetcellen geïnjecteerd worden. Door de liposuctie, de centrifugatie en het injecteren wordt het vetweefsel immers onderworpen aan mechanische krachten en een periode van zuurstoftekort, waardoor vetcellen kunnen afsterven.

Zeefjes

Vetcellen zijn niet de enige cellen in het vetweefsel. Ook stamcellen en onrijpe vetcellen worden gevonden en zij spelen zeker hun rol. Dit vormde de basis van het onderzoek op de dienst plastische en reconstructieve heelkunde van het UZ Brussel dat gevoerd werd door dr. Maxim Geeroms en dr. Barbara Craggs onder leiding van professor Moustapha Hamdi. Om het slinkende ingespoten vet en de extra lipofillingsessies te vermijden, onderzochten zij of met een extra stap tijdens de operaties het geschikte vetweefsel zou kunnen geselecteerd worden. Door het opgezogen vetweefsel te scheiden met zeefjes, verkregen zij 4 verschillende groepen met groepjes vetcellen en andere cellen van verschillende grootte. Nadien werd het percentage levende cellen geteld in elke groep. Ook de samenstelling van elke groep werd microscopisch bestudeerd.

Verder onderzoek

Ondertussen werd in het laboratorium van de VUB vetweefsel bestudeerd van 7 patiëntes die liposuctie ondergingen. Uit deze eerste testen blijkt dat, na het zeven van het vet, er een licht voordeel is voor de stalen met de kleinste groepjes van samengeklitte cellen wat betreft het aantal levende cellen. Ook de samenstelling van het vetweefsel is niet helemaal dezelfde in de grote en de kleine groepen. Dr. Geeroms benadrukt echter dat er voorlopig geen grote verschillen werden gevonden: "Door dit beperkte aantal van 7 patiëntes kunnen wij op dit moment nog geen interessante conclusies trekken. We merken wel een trend, zowel in de leefbaarheid van de cellen als in de compositie van het staal, maar verder onderzoek blijft noodzakelijk. Hiervan zal dan afhangen of wij het zeven en selecteren van vetweefsel tijdens de operatie kunnen aanraden voor een optimaal resultaat waardoor we bijkomende lipofillingsessies en de verbonden risico's en kosten kunnen vermijden."
