De (totale) knieprothese, een delicaat beeldhouwwerk?

Gilbert en zijn versleten knieGilbert heeft al jaren last van zijn knie. Vroeger hield hij enorm van voetballen, maar nu raakt hij niet meer vooruit. De tocht van de zetel naar de televisie is een hel. Pijnstillers bieden geen soelaas meer. Zijn vrouw dringt aan dat hij eindelijk eens naar een dokter zou gaan; een knieprothese blijkt onvermijdelijk…

Maar hoe moet de chirurg zijn knieprothese plaatsen? En als de prothese niet perfect geplaatst wordt, beïnvloedt dat hoe Gilbert zich voelt na de operatie?

De totale knieprotheseDe (totale) knieprothese is de ultieme oplossing voor een knie die geplaagd wordt door artrose, een “versleten” knie. Hierbij wordt het kapotte gewrichtsoppervlak weggesneden en vervangen door twee stukken metaal: één voor het bovenbeen en één voor het onderbeen. Tussen die twee delen zit  ten slotte een stuk plastic, die de functie van het kraakbeen als schokdemper overneemt. De vorm van de knieprothese sluit nauw aan bij een gewone knie: de prothese van het onderbeen is vlak, een plateau, terwijl de prothese van het bovenbeen uit twee bolvormen bestaat die over het plateau van het onderbeen rollen wanneer je je been plooit of strekt, de condylen.

Om de plaatsing van de knieprothese te bepalen meet de chirurg enkele hoeken ter hoogte van het been en de knie. Hieronder de twee belangrijkste:

  1. Een eerste hoek is de hoek tussen het bovenbeen en het onderbeen, met andere woorden of je X- of O-benen (of ‘rechte’ benen) hebt. Artrose, of het slijten van een knie, is eigenlijk niets anders dan het kraakbeen van de knie (dat fungeert als schokdemper) dat verdwijnt en dus zijn functie niet meer kan uitvoeren. Vaak gebeurt dit meer aan de ene dan aan de andere helft  van de knie en komt het been zo ‘scheef’ te staan. Hierdoor ondervindt de helft van de knie die al aangetast was nog grotere lasten, verdwijnt het kraakbeen er nog sneller en belandt men in een vicieuze cirkel waarbij het been steeds schever komt te staan.

Tegenwoordig tracht de chirurg er voor te zorgen dat de prothese zodanig geplaatst wordt dat het been na de operatie mooi recht staat. Hierbij wordt de druk zo egaal mogelijk over het volledige knieoppervlak gespreid en men gaat ervan uit dat dit de beste positie is. Men heeft echter nog onvoldoende onderzocht of je ongestraft van een ‘scheef’ been een ‘recht’ been mag maken. Misschien zijn de spieren en ligamenten zodanig aan deze ‘scheefstand’ aangepast dat men het been beter een klein beetje scheef laat staan (en bewandelt men als het ware een gulden middenweg)om klachten te vermijden na de operatie.

  1. Een tweede hoek beschrijft de rotatie van het gewrichtsoppervlak/de prothese ten opzichte van het bovenbeen. Hierbij moet je je voorstellen dat je langs onder kijkt op het bovenbeen. Het eerste dat je ziet, is het gewrichtsoppervlak van het bovenbeen (na de operatie is dat de bovenbeensprothese). Zoals je weet, bestaat dit als het ware uit twee bollen. Als je deze twee bollen tegen elkaar vasthoudt, kun je deze met of tegen de wijzers van de klok draaien; je roteert het gewrichtsoppervlak/de prothese.

Tijdens de operatie tracht men de prothese zodanig te plaatsen dat deze rotatie overeenkomt met die van de ‘gemiddelde’ knie.

Hier drukt het citaat van Kirkley & Griffin de vinger op de wonde. Voor deze metingen (en ook voor de andere metingen bij de knieprothese) zijn er namelijk een heleboel varianten beschreven, de één al beter dan de ander. Vaak zijn deze niet gevalideerd en weet niemand precies hoe belangrijk ze zijn. In onze studie hebben we gekozen om die hoeken eruit te kiezen die door eerder onderzoek nog niet in diskrediet gebracht werden en waarvan een consensus van hun nut bestaat. Vervolgens onderzochten we of deze hoeken wel betrouwbaar zijn en of ze bijgevolg gebruikt konden worden in onze studie (en of chirurgen ze überhaupt betrouwbaar kunnen meten).

ResultatenUit dit onderzoek bleek dat alle metingen betrouwbaar bepaald kunnen worden, zodat deze vervolgens vergeleken konden worden met de patiëntresultaten. Hierbij werd de plaatsing van de prothese op verschillende manieren vergeleken met de patiëntresultaten voor en na de operatie.

Over het algemeen wordt de knieprothese best geplaatst volgens de bestaande richtlijnen, waarbij het opvalt dat de rotatie van de bovenbeensprothese preferentieel niet te veel afwijkt van de rotatie van het gewrichtsoppervlak voor de operatie. Aan de hoek tussen het boven- en het onderbeen mag daarentegen wel veel gesleuteld worden tijdens de operatie, zolang het been na de operatie maar ‘recht’ staat (met een marge van een aantal graden).

Het meest opmerkelijke resultaat viel te rapen bij de rotatie van de bovenbeensprothese. Door onderzoek op patiënten met blijvende kniepijn na hun operatie wist men reeds dat te veel inwendige rotatie van de prothese nadelig is voor de patiënt. Wat betreft uitwendige rotatie was dit een pak minder duidelijk. Om dus zeker geen inwendige rotatie te bekomen, beslist de chirurg– indien hij twijfelt over de positie van de prothese tijdens de operatie – soms om de prothese in iets meer uitwendige rotatie te plaatsen; als een soort marge. Uit dit onderzoek is echter gebleken dat ook uitwendige rotatie van de bovenbeensprothese nefast is voor de patiënt. Uitwendige rotatie mag dus niet langer als een veilige zone beschouwd worden, de chirurg moet de rotatie precies goed hebben.

Gilbert en zijn prothesePraktisch betekent dit dat de plaatsing van Gilberts knieprothese wel degelijk belangrijk is en dat de chirurg tijdens de operatie niet zomaar om het even wat mag doen. De prothese wordt idealiter binnen het referentiekader geplaatst, waarbij het been recht gezet wordt en de bovenbeensprothese  niet te veel in inwendige, maar dus ook niet te veel in uitwendige rotatie geplaatst mag worden.

Wanneer men deze regels respecteert kan Gilbert met een beetje geluk opnieuw twintig jaar pijnvrij supporteren voor zijn favoriete  voetbalploeg…
