The influence of subthalamic nucleus stimulation on the pragmatic language production in Parkinson's disease

De ziekte van Parkinson en diepe hersenstimulatie

Onderzoek naar de invloed op de pragmatieke taalproductie van patiënten

„Diepe hersen-wadde?” is de reactie die ik van de meeste geïnteresseerden te horen kreeg wanneer ze me vroegen waarover ik mijn thesis schreef. Toegegeven, diepe hersenstimulatie van de nucleus subthalamicus (of STN-stimulatie) is geen kant-en-klare taal. Het is vrij specifiek vakjargon en wordt nagenoeg enkel gebruikt in de medische wereld, maar wat het is, wordt later duidelijk. Daarenboven is dit een interfacultaire studie van de Universiteit Gent tussen enerzijds de faculteit Letteren en Wijsbegeerte en anderzijds de faculteit Geneeskunde en Gezondheidswetenschappen. Dit draagt ertoe bij dat de materie en de verwerkte data vanuit een ander standpunt werd benaderd en als dusdanig tot nieuwe inzichten. Voor we dieper ingaan op de studie zelf moeten we eerst kort de pathologie uitleggen, of, met andere woorden, toelichten wat de Ziekte van Parkinson is nu juist inhoudt.

De Ziekte van Parkinson: meer dan alleen tremorDe ziekte van Parkinson is de tweede meest voorkomende neurodegeneratieve aandoening waarbij cellen in het centrale zenuwstelsel afsterven en waar wereldwijd 4 tot 6 miljoen mensen aan lijden. Volgens een studie zijn er in België momenteel zo’n 30,000 patiënten bekend. Parkinson wordt gekarakteriseerd door een asymmetrische degeneratie van het zogenaamde eiwit dopamine in de substantia nigra en de basale ganglia, twee subcorticale structuren in onze hersenen. Al vanaf het begin zien we dat één hemisfeer meer aangetast is, wat uiteindelijk bijdraagt tot allerlei dysfuncties in een verder stadium van de ziekte. Van zodra de hersenen het dopamineverlies niet langer kunnen verwerken en tegenwerken, treden er motorische problemen op zoals tremor en stijfheid, maar ook spraak- en stemproblemen.

Hoewel de motorische dysfuncties het best gekend zijn - omdat ze het meest bestudeerd worden - zijn er ook non-motorische symptomen, problemen op het vlak van cognitie en taal. Zo vertoonden patiënten bijvoorbeeld geheugen- en benoemingsproblemen, hebben ze moeite om aandachtig te zijn en gebruiken ze grammaticaal minder complexe constructies. Of Parkinson al dan niet een invloed had op de pragmatieke taalvaardigheid van patiënten werd tot voor kort achterwege gelaten. Onder pragmatisch taalgebruik verstaan we alle vaardigheden die nodig zijn om een normale en goede conversatie te voeren met iemand anders. Een pragmatische taaldysfunctie heeft dus vergaande gevolgen en is dus belangrijk om verder in detail te bestuderen als we Parkinsonpatiënten hun socio-economische relaties willen helpen behouden of ze opnieuw willen re-integreren in de maatschappij. Uit recente onderzoeken blijkt dat de ziekte van Parkinson wel degelijk een impact heeft op een patiënt zijn/haar pragmatieke taalcompetenties, voornamelijk in het begrijpen en verwerken van nieuwe informatie. Maar wordt dit probleem gecompenseerd wanneer de patiënt diepe hersenstimulatie krijgt?

Diepe hersenstimulatie: for better or for worse?Bij een diepe hersenstimulatie worden er twee elektroden in de hersenen ingeplant wat ons toelaat om de nucleus subthalamicus - een belangrijke subcorticale kern - te gaan stimuleren. Hoewel onderzoekers al bewezen hebben dat het de motorische dysfuncties verbetert, blijft de impact op taal relatief ononderzocht. Aangezien enkel de invloed op lexicale en grammaticale vaardigheden al bestudeerd is, besloten we een voorstudie af te nemen waar de data van een patiënt geanalyseerd werd om te kijken of al dan niet impact merkbaar was. De analyse toonde aan dat diepe hersenstimulatie de pragmatieke taalproductie van de patiënt beïnvloedde, meer bepaald dat rechterhemisfeer-stimulatie een negatieve resultaat had. Zo herhaalde ze vaker haar uitingen en waren haar antwoorden minder samenhangend. Maar omdat we slechts de data van één patiënt bestudeerden, was het niet duidelijk of de negatieve invloed volledig toe te schrijven is aan de stimulatie of aan het feit dat de patiënt links motorisch gelateraliseerd was, met andere woorden dat de rechter hemisfeer het meest aangetast was door de ziekte van Parkinson.

Om deze hypothese te toetsen, analyseerden we de data van 18 patiënten waarvan de helft links motorisch gelateraliseerd was en de andere helft rechts. Hoewel de gebruikte test weinig significante parameters opleverde, was het duidelijk dat diepe hersenstimulatie wel degelijk de patiënten hun pragmatieke vaardigheden beïnvloedde. Zo zagen we niet alleen dat de hersenen rechtshemisferische processen - zoals bijvoorbeeld je mening geven over iets - linkshemisferisch opvingen maar dat die daarenboven betere resultaten opleverde tijdens de stimulatie. De analyse van de data toont dus aan dat er geen sprake is van ofwel een positieve ofwel een negatieve invloed maar van beide. Enerzijds zagen we bij een aantal patiënten een negatieve invloed op het aantal herhalingen in de antwoorden, maar anderzijds bevorderde de stimulatie het gebruik van verbindingswoorden wat de samenhorigheid ten goede kwam. Bovendien wijst het onderzoek aan dat de organisatie van pragmatieke taal complexer is dan dat voordien werd gedacht en dat het niet uitsluitend rechts hemisferisch is maar dat er ook participatie van de linkerhemisfeer is.

Wat nu?Hoewel de test die we gebruikten, namelijk de Nijmeegse Pragmatiektest, aan zijn oorspronkelijk doel beantwoordt als complementair hulpmiddel, blijft het niettegenstaande een test die dient om de pragmatieke taalcompetenties van kinderen met een taalprobleem te evalueren. Dit draagt ertoe bij dat sommige parameters onbruikbaar waren voor dit onderzoek waar de te toetsen populatie verschilt van de eigenlijke doelgroep. Bovendien wordt het onderzoek naar pragmatieke taalproblemen steeds belangrijker. De volgende stap is dus het opstellen van een nieuwe gestandaardiseerde Nederlandstalige pragmatiektest die specifiek ontworpen is om mensen met een neurodegeneratieve aandoening te evalueren. Dit zou niet enkel voordelig zijn voor het medisch personeel, maar ook voor de patiënt aangezien hij of zij beter geholpen kan worden.

Kortom, hoewel het onmogelijk is om een concreet antwoord te geven op de vraag wat de invloed van diepe hersenstimulatie is op de pragmatieke taalproductie van een patiënt, levert dit onderzoek toch zijn bijdrage aan de wetenschap. Niet enkel toont de data aan dat het klassieke model van de pragmatieke taalorganisatie herzien moet worden maar ook dat er nood is aan een gestandaardiseerde Nederlandstalige pragmatiektest.
