Zinvol voorschrijven bij ouderen in de laatste levensfase

De verlengde levensverwachting en bijgevolg onoverkomelijke vergrijzing van de populatie, leidt tot het zich ontpoppen van een complexe zorgvraag in het geriatrisch landschap. Zo is deze oudere en vaak kwetsbare populatie opmerkelijk gevoeliger voor ziekte, met daaruit voortvloeiend een abundant geneesmiddelengebruik. Uit studies blijkt immers dat maar liefst 50% van de 65-plussers 5 of meer geneesmiddelen inneemt. De combinatie van voorgaande factoren maakt oudere mensen opmerkelijk gevoelig voor de nadelige effecten van medisch handelen. Als gevolg hiervan heerst er een internationale bezorgdheid over het veel voorkomend onoordeelkundig voorschrijven van medicatie bij ouderen.

Het eeuwenoud adagium “La médecine c’est guérir parfois, soulager souvent, consoler toujours”, wellicht aan de hand van Ambroise Paré, kan beschouwd worden als een rode draad in de zorg voor de oudere in de laatste levensfase. Geneeskunde wordt dan opnieuw verheven tot op het niveau van de ‘genees-kunst’, waarbij het aloude advies van Hippocrates, “primum non nocere”, hoog in het vaandel gedragen wordt. Bij de oudere patiënt in de laatste levensfase kent men immers een hoofdrol toe aan het waken over het comfort met oog op het behouden van een zekere levenskwaliteit, en ziet men af van invasieve levensverlengende interventies. De zorg tijdens de laatste levensfase is bijgevolg een kwestie van individueel maatwerk, waarbij de arts rekening dient te houden met de wensen en noden van de patiënt bij het opstarten van een behandelplan.

Zo zal het in de laatste fase nodig zijn om na te gaan welke geneesmiddelen kunnen worden stopgezet, omdat ze niet langer als zinvol beschouwd kunnen worden. Het is een hele opgave voor de behandelend arts om deze opdracht tot een goed einde te brengen. Enige terughoudendheid vanuit artsenperspectief hierbij kan ondermeer verklaard worden vanuit het feit dat de arts niet wil dat de patiënt en diens familie zich in de steek gelaten voelen. Bovendien is er voor het stopzetten van medicatie in de laatste levensfase weinig evidentie in de wetenschappelijke literatuur, wat logischerwijze ook een struikelblok vormt voor de behandelend arts. Uit studies blijkt echter wel dat het overvloedig voorschrijven van ondermeer slaapmedicatie, antidepressiva en medicatie voor hoge bloeddruk wel degelijk een reëel probleem vormt: ouderen zijn gevoeliger voor de nadelige effecten van deze medicatie en als gevolg hiervan zijn bijvoorbeeld valpartijen en verwardheid veel voorkomende problemen. Uit verschillende studies is dan gebleken dat bepaalde geneesmiddelen in geselecteerde gevallen en met de nodige voorzichtigheid, kunnen worden stopgezet. Dit geldt voor medicatie met een effect op het centraal zenuwstelsel (antipsychotica, benzodiazepines, anti-depressiva en cholinesterase-inhibitoren),

cardiovasculaire medicatie (antihypertensiva, cholesterolverlagende middelen, salicylaten en nitraten), medicatie die een invloed uitoefent op het gastro-intestinaal en endocrien stelsel (laxativa, protonpompinhibitoren, oraal hypoglycemiërende middelen en insuline) en tot slot medicatie die een effect heeft op het botmetabolisme (bisfosfonaten). Voor het stopzetten van geneesmiddelen die niet langer als zinvol kunnen worden beschouwd, geldt het ‘stop most, reduce dose’ principe.

Daarnaast is het in de laatste levensfase ook belangrijk, met oog op het maximaliseren van de levenskwaliteit, om een adequate pijn-en symptoomcontrole te voorzien. Ook dit blijkt vaak niet eenvoudig omdat het voor de arts niet altijd mogelijk is om zich te verplaatsen in het lijden van de patiënt, die op dat moment vaak moeilijk klachten verwoordt. Uit een studie blijkt dan ook dat er vaak onvoldoende controle is van angst en pijn. Daarenboven is het zo dat ook voor het opstarten van medicatie tijdens de laatste 48 uur van het leven van de oudere, er, ondanks wijdverbreid gebruik, vaak een gebrek aan evidentie heerst. Voor het opstarten van dergelijke noodzakelijke geneesmiddelen, staat het ‘start low, go slow’ principe centraal.

Concluderend kunnen we stellen dat er door een gebrek aan harde wetenschappelijke evidentie, geen waterdichte aanbevelingen geformuleerd kunnen worden voor deze oudere populatie in de laatste levensfase, zowel wat betreft het stopzetten van overbodige medicatie als het opstarten van medicatie die als zinvol beschouwd wordt. En toch is het net dié groep van de bevolking die het meeste medicijnen inneemt. Deze informatiekloof tussen de op wetenschappelijk onderzoek gestoelde evidentie en de reeële setting, brengt met zich mee dat het voorschrijven van geneesmiddelen bij ouderen vaak tekort schiet qua uniformiteit. Gezien de vergrijzingsgolf, zal men in de toekomst steeds meer geconfronteerd worden met deze hiaten en valkuilen. Het is dan ook van essentieel belang om studies bij de oudere populatie op touw te zetten die beantwoorden aan de huidige tekortkomingen. De evidentie die hieruit voortvloeit, moet gezien worden als een vertrekpunt voor een holistische beandering van de oudere patiënt. Door dergelijke multidisciplinaire geïntegreerde benadering te hanteren, kan men komen tot oordeelkundig en bovenal zinvol voorschrijfgedrag, op maat van de patiënt.
