“My mom made chocolate pudding. I refused to eat. She said ‘since when do you not eat puddings?’I'm proud of my self-control. I am pure. I am close to a perfect body and a perfect soul.”

Het perfecte lichaam en de perfecte ziel, stiekem dromen we er allemaal wel eens van. Maar wat is ‘perfectie’? Wanneer dit gelijkgesteld wordt aan het getal dat op de weegschaal verschijnt, kan dit een zware impact hebben op zowel de fysieke als de mentale gezondheid.

Het citaat hierboven is terug te vinden op een pro-ana website. Pro-ana staat voor 'pro-anorexia': een online beweging die eind jaren ‘90 ontstond, door het toenemende internetgebruik aan populariteit won en anorectische meisjes uit de isolatie hielp.

Anorexia is een eetstoornis die zich kenmerkt door het verlangen steeds magerder te zijn. Het lichaamsgewicht wordt een ware obsessie en men hongert zichzelf uit. Hoe mager ook, men blijft zichzelf te dik vinden. Het afvallen is als een verslaving, er kan niet meer mee gestopt worden. De gevolgen kunnen nefast zijn: van alle psychiatrische stoornissen, hebben eetstoornissen het vaakst een dodelijke afloop.

Hoewel de medische wereld anorexia omschrijft als een ernstige ziekte, wordt het door anorectici soms helemaal anders beleefd. In onderzoek en discussie omtrent anorexia, is de stem van de patiënt echter vaak afwezig. Hoe ervaren zíj hun ‘eetstoornis’?

Pro-ana fora en blogs bieden een unieke inkijk in de leefwereld van het anorectische individu. Gevoelens, gedachten en ervaringen omtrent het eetgedrag worden hier geventileerd. Voor velen is een pro-ana website de enige plaats waar ze terecht kunnen. Vaak voelen ze zich niet begrepen door familie en vrienden, of durven ze hen niet te vertellen over de eetstoornis. Hierdoor kan anorexia een eenzame en isolerende ervaring zijn. Het anonieme internet biedt een plaats waar ze terecht kunnen voor steun, begrip en advies.

In recent onderzoek werd nagegaan hoe de beleving van anorexia wordt weergegeven op het internet. Uit de studie bleek dat anorexia –oftewel ‘Ana’– verschillende betekenissen krijgt toegekend door het individu dat blogt op de pro-ana website, zowel positieve betekenissen als negatieve. De één apprecieert de symptomen; de ander haat de eetstoornis en verlangt ernaar terug gezond te zijn. ‘Ana’ kan zowel een vriendin zijn, als een vijand. Soms zelfs beide tegelijk –of hoe er genoten kan worden van het lijden.

Getuigenissen op de pro-ana sites zijn beladen met negatieve gevoelens tegenover zichzelf en het eigen lichaam. Wanneer walging opborrelt telkens men in de spiegel kijkt, is de anorexia als een oplossingspoging. Het ‘slank zijn’ houdt de belofte in van ‘gelukkig zijn’. Heel vaak draait het om het willen veranderen van zichzelf en anorexia hoort bij dit proces. Het gaat om het hebben van een bepaalde uitstraling en identiteit. Het kan deel uitmaken van wie je bent of het is karakteristiek aan je levensstijl.

Anorexia betekent ook zelfcontrole: het in staat zijn het eigen lichaam te controleren en sterker te zijn dan de fysiologische behoeftes. Het vermageren geeft zelfvertrouwen en doet een persoon sterk, puur en zelfs euforisch voelen.

Sommigen omschrijven het als een manier van omgaan met bepaalde gevoelens en gebeurtenissen zoals stresssituaties, depressie en misbruik.

Ten slotte, kan de eetstoornis een vorm van non-verbale communicatie zijn om naasten te informeren dat het niet goed gaat. Sommigen willen meer zorg en aandacht ontlokken, anderen daarentegen zien de anorexia net als een manier om te verdwijnen.

“Jesus Christ, I know that this is killing me and I can’t stop.”

Maar anorexia wordt ook ervaren als een constante strijd. Individuen kunnen wel degelijk ziektebesef hebben en weten dat bepaalde gedragingen – zoals het extreem vasten, laxeren of het zelfopgewekt braken – schadelijk zijn voor het lichaam. De anorexia is een probleem. Het is bovendien zinloos en frustrerend want het einddoel wordt nooit bereikt: men voelt zich nooit mager genoeg. Maar al zou men ermee willen stoppen, vaak lijkt dit niet te lukken. ‘Ana’ is in controle en wordt omschreven als “diegene die de broek draagt in de relatie”.

“Pretty girls don’t eat.”

De beleving van anorexia –zoals het wordt weergegeven op de pro-ana websites– is voor iedereen verschillend en op zijn minst intens te noemen. Lichaamsontevredenheid, onzekerheid, zelfhaat en depressieve gevoelens kleuren de getuigenissen. Een lichaam dat niet perfect is, lijkt moeilijk te aanvaarden. Immers: “An imperfect body reflects an imperfect person”. Het is een quote die op verschillende pro-ana websites wordt teruggevonden. Het lijkt bovendien een stereotiep beeld te zijn in onze hedendaagse westerse maatschappij. Iemand met een slank, jong lichaam wordt gezien als mooi, gezond, intelligent en succesvol. Dikke mensen daarentegen krijgen een negatieve stereotypering: ze zijn lui en ontbreken aan zelfdiscipline. Maar anorexia draait om zoveel meer dan het uiterlijk. Het heeft een functie; het helpt bij het bereiken van bepaalde doelen; en het doet een persoon gewoonweg goed voelen. Zoals topmodel Kate Moss, icoon van de erg magere “heroin chic”-look uit de jaren ‘90 het stelde: “nothing tastes as good as skinny feels”. Het is niet verwonderlijk dat behandeling moeilijk is als een ‘stoornis’ zo goed kan voelen.

Het onderzoek van getuigenissen op pro-ana blogs en fora draagt bij tot het beter begrijpen van het anorectische individu. Het biedt diepgaander inzicht in processen die een rol kunnen spelen bij het behouden van de eetstoornis. Resultaten van de studie kunnen dienen als een inspiratie voor hulpverleners en bijdragen tot een betere behandeling. Want ondanks heel wat wetenschappelijk onderzoek naar deze eetstoornis, blijft het vinden van een effectieve behandeling een uitdaging.
