Onderzoek naar levenskwaliteit bij patiënten met chronisch leverlijden, pre- en posttransplantatie

Een nieuwe lever, een nieuw leven.

“Is life worth living? It all depends on the liver.” Dokter William James wist het al lang. Elk jaar krijgen in België 220 mensen een nieuwe lever. De vraag is echter, in hoeverre deze mensen nog een toekomst hebben.

Het orgaan
De lever van een volwassen mens weegt ruim anderhalve kilo en is een van de grootste organen van het menselijke lichaam. Hij wordt ook wel eens de afvalverwerker van het lichaam genoemd. De talloze functies van de lever omvatten onder  andere het afbreken van medicatie en alcohol, wat het bloed zuivert.

Soms echter, wordt de lever ziek. Deze ziektes omvatten onbehandelbare leverinfecties, overmatig alcoholgebruik, leverkanker en erfelijke leveraandoeningen. De voorbije jaren werd levertransplantatie de behandeling bij uitstek bij ziektes waarbij de lever niet meer te redden valt. Orgaantransplantatie is een effectieve ingreep voor patiënten met vergevorderde chronische orgaangebonden ziektes. Dit geldt voor de meeste belangrijke organen, waaronder ook de lever. Dit zal voor vele mensen een ver-van-mijn-bedshow lijken (gelukkig), maar het is een interessant gegeven waar niet veel over is geweten.

Wacht eens even
Voor men een transplantatie ondergaat, moeten patiënten gemiddeld een aantal maanden wachten op een geschikt orgaan. Als een orgaan niet zorgvuldig wordt uitgekozen voor elke patiënt apart, is er een hoge kans dat het immuunsysteem het nieuwe orgaan zal beschouwen als een indringer. En indringers, die worden verstoten.

De tijd dat de zieke mensen moeten wachten op hun nieuwe orgaan is meestal lang en stressvol. Er wordt van hen verwacht dat ze stand-by zijn, dag en nacht. Eenmaal er een perfect passend orgaan beschikbaar is, moet het plots allemaal heel snel gaan. Je kan je wel inbeelden dat dit proces veel energie vraagt van de patiënten en hun families.

I will survive! … and still be happy
Een dertigtal jaar geleden werd het succes van deze operatie vooral gemeten aan de hand van de overleving. De overlevingskans was toen slechts 20%. Tegenwoordig leeft vijf jaar na de ingreep bijna 3/4de van de patiënten nog.

Vandaag de dag zijn de chirurgische technieken en de zorg in het ziekenhuis zo goed dat er nu ook naar een andere manier wordt gekeken om het succes van de operatie te beoordelen. De laatste jaren werd de levenskwaliteit na een levertransplantatie belangrijker: hoeveel heeft de patiënt nog aan zijn dagelijkse leven, zowel fysiek als emotioneel?

Wij onderzochten of patiënten zich na de operatie beter voelden en hoe dit evolueerde in de loop van de tijd.

Het nieuwe begin
Vanaf drie maanden na de operatie voelen patiënten zich zowel fysiek als emotioneel stukken beter. We gebruiken de grens van drie maanden als tijd waarin de patiënt op zijn positieven kan komen.

De gemiddelde patiënt brengt de eerste maand na de transplantatie in het ziekenhuis door, waarvan de eerste week op de afdeling intensieve zorgen. Een maand lang in een ziekenhuiskamertje doorbrengen beperkt de beweging van de patiënt. Dit maakt de grote vooruitgang op fysiek vlak verbazingwekkend. Zo blijft alles beteren tot ruim twee jaar later, met de sterkste verbetering in de eerste zes maanden na de transplantatie.

Een opvallend weetje: de tijd die de patiënt doorbrengt op intensieve zorgen is zeer belangrijk voor de fysieke evolutie. Hoe langer men intensieve zorgen nodig heeft, hoe slechter voor de fysieke verbetering. Natuurlijk hebben enkel de heel zieke patiënten lange intensieve zorgen nodig.  

Emotioneel worden deze patiënten zeer goed ondersteund. Enkele maanden op de wachtlijst doorbrengen doet wat met een mens (en zeker met zijn familie). Altijd stand-by blijven, terwijl men steeds zieker wordt, is geen pretje. De verlossing is dan ook groot. Dit draagt bij tot de emotionele verbetering. De evolutie van de emotionele stand van zaken verloopt ook positief, met een rustig stijgende lijn.

Be-leef je ziekte
De laatste jaren kijken artsen vaker naar hoe de patiënt zijn ziekte beleeft. Onder deze ziektebelevingen verstaan we: hulpeloosheid, acceptatie en ziektevoordelen. Na een levertransplantatie zijn de patiënten een stuk minder hulpeloos, ze voelen zich weer zelfstandiger. Niet langer zijn ze afhankelijk van familie voor dagdagelijkse eenvoudige taken. De acceptatie van hun situatie wordt beter, patiënten aanvaarden hun ziekte en hebben vrede met de toekomst. Ze gaan er ook de voordelen van inzien. Ik zie je al denken: “Welke voordelen vallen er hier nu in godsnaam in te zien?” Wel, ik kan je vertellen dat deze mensen zeer veel aandacht van hun omgeving krijgen, ze worden dikwijls extra in de watten gelegd. Dit noemt men dan fijntjes de ‘ziektevoordelen’.

De evenwichtige kijk op de zaak zorgt ervoor dat deze patiënten genieten van een totaalpakket aan zorg. De verschuiving van een louter medische naar een meer patiënt- en omgevingsgerichte opvolging heeft duidelijk zijn vruchten afgeworpen.

Of deze patiënten nog een toekomst hebben? Wees maar zeker. En deze is nog rooskleurig ook.

We kunnen besluiten dat de meeste mensen die een levertransplantatie ondergaan beter worden over de hele lijn. Ondanks de immense impact die de ingreep heeft op het leven van de patiënt. Beter gezegd: een nieuwe lever, een nieuw leven.
