Patiëntgerapporteerde uitkomsten van vrouwen met endometriose

Endometriose is een chronische gynaecologische aandoening die gekenmerkt wordt door de aanwezigheid van endometriumweefsel buiten de uterus (Kennedy et al., 2005). Dit induceert een chronische ontstekingsreactie die kan leiden tot laesies op de bekkenorganen, cysten ter hoogte van de ovaria, fibrose en de vorming van verklevingen (Kennedy et al., 2005). Endometriose is één van de meest frequent voorkomende gynaecologische aandoeningen en treft ongeveer 10% van de vrouwen tijdens hun reproductieve levensjaren (Eskenazi en Warner, 1997).   
Veelvoorkomende klachten bij vrouwen met endometriose zijn chronische onderbuikpijn, cyclisch variërende pijn (zoals dysmenorroe, dyspareunie, dyschezie en dysurie) en subfertiliteit (Kennedy et al., 2005; Jackson en Telner, 2006; Meissner et al., 2010). Naast de fysieke impact van endometriose is er ook een belangrijke psychosociale impact op het leven van vrouwen. Voorgaand onderzoek vond geen relatie tussen de graad van endometriose en de mate waarin patiënten endometrioseklachten (met name pijn) ervaren (Facchin et al., 2016). De vraag of er nog andere factoren zijn dan de graad van endometriose die de symptoomervaring en psychosociaal welzijn van vrouwen met endometriose kunnen verkaren, dringt zich op.  
Eén van de factoren die hier mogelijks een invloed op kan hebben, is de psychologische weerbaarheid of ‘sense of coherence’ (SOC) van de vrouw (Antonovsky, 1987). SOC verwijst naar de mogelijkheden om met stressveroorzakende factoren om te gaan (Antonovsky, 1987). Uit onderzoek bij vrouwen met andere chronische                                                                 
aandoeningen, is reeds gebleken dat een sterke SOC gerelateerd is aan een goede fysieke en psychosociale gezondheid en kwaliteit van leven (KvL) (Motzer et al., 2003; Sarenmalm et al., 2013). De hypothese dat de SOC van vrouwen met endometriose een invloed kan hebben op hun psychosociaal welzijn werd nog niet eerder onderzocht. Om die reden werd een monocentrische, observationele, vragenlijst studie uitgevoerd die data verzamelde in het Leuven University Fertility and Endometriosis Center (LUFC), België. Er werden bij 64 vrouwen medische karateristieken alsook vier patiëntgerapporteerde uitkomsten (PRO’s) bevraagd, namelijk: psychologische weerbaarheid, endometriosespecifieke KvL, algemene KvL en subjectieve gezondheidstoestand. Aan de hand van correlaties en een hiërarchische regressieanalyse werd de relatie tussen deze factoren nagegaan.  
De algemene KvL en de subjectieve gezondheidstoestand werden door de totale steekproef (64 vrouwen) gerapporteerd. Vrouwen met endometriose rapporteerden een mediane algemene KvL van 70.0/100, de gemiddelde subjectieve gezondheidstoestand was 63.3/100. Verder vulden 62 vrouwen (96.9%) de vragenlijsten omtrent endometriosespecifieke KvL en psychologische weerbaarheid volledig in. Ze rapporteerden een mediane score van 59.0/120 voor endometriosespecifieke KvL en een gemiddelde psychologische weerbaarheid van 59.5/91. Deze preoperatieve scores op de PRO’s bleken niet significant gecorreleerd te zijn aan de graad van endometriose.  
Elke preoperatieve PRO werd gecorreleerd aan de verschillende endometrioseklachten. Vrouwen die onderbuikpijn buiten de menstruatie ervaarden, scoorden significant lager op

zowel algemene KvL, endometriosespecifieke KvL als op SOC. Verder bleek de aanwezigheid van braken of misselijkheid gecorreleerd te zijn aan een significant slechtere algemene KvL en subjectieve gezondheidstoestand. Twee andere PRO’s die eveneens gecorreleerd waren aan een slechtere subjectieve gezondheidstoestand waren nycturie en dyschezie. Als laatste werd een verminderde endometriosespecifieke KvL niet enkel gezien bij vrouwen met onderbuikpijn buiten de menstruatie, maar ook bij vrouwen met metrorragie en hematurie.  
Het hiërarchisch regressiemodel werd opgebouwd door na te gaan voor welke demografische variabelen, medische patiëntenkarakteristieken en endometrioseklachten er gecorrigeerd moest worden. Uit deze analyses bleek dat geen enkele endometrioseklacht significant gecorreleerd was aan het psychosociaal welzijn. Opleidingsniveau was significant gecorreleerd aan algemene KvL en endometriosespecifieke KvL. Verder was een partnerrelatie eveneens significant gecorreleerd aan algemene KvL. Beide demografische variabelen werden in het model meegenomen als mogelijke ‘confounders’. Graad van endometriose werd hier eveneens aan toegevoegd. Wanneer er gecorrigeerd werd voor deze graad en de ‘confouders’, bleek SOC een significante voorspeller te zijn voor endometriosespecifieke KvL, maar niet voor algemene KvL en subjectieve gezondheidstoestand.   
Aangezien een sterke psychologische weerbaarheid een positieve invloed heeft op het psychosociaal welzijn van deze vrouwen, is het van belang dat hier binnen de endometriosezorg meer aandacht aan besteed wordt. Het is dus aanbevolen om 
interventies te implementeren binnen endometriosezorg om de psychologische weerbaarheid van deze vrouwen te verbeteren. Op deze manier kan er gestreefd worden naar meer patiëntgerichtheid om zo te voldoen aan de steeds belangrijker wordende kwaliteitsdimensie van de gezondheidszorg (IOM, 2001).
