Slaap op je beide oren: de ergotherapeut als slaapcoach

42% van de Vlamingen geeft aan niet tevreden te zijn over zijn slaap, 33% spreekt zelfs over echte slaapproblemen. Kijk in je eigen omgeving en je merkt dat slaapproblemen geen ver-van-je-bedshow zijn. Toch wordt het belang van slaap in onze samenleving nog steeds onderschat. Mensen die veel slaap nodig hebben, worden als zwak aanschouwd. Ook de medische wereld heeft onvoldoende oog voor de behandeling van slaapproblemen. Nochtans zijn de gevolgen van onvoldoende slaap niet te onderschatten.

Slapeloosheid is niet zo’n onschuldige aandoening als vaak wordt gedacht. De kans op depressie bij personen met aanhoudende insomnia ligt maar liefst 14 keer hoger dan bij individuen met een gezonde slaap. Insomnia verhoogt eveneens het risico op hartaandoeningen, op het ontstaan van kanker, op gewichtstoename en op de aanwezigheid van infecties en ontstekingen. 

In Vlaanderen wordt nog al te vaak naar slaapmedicatie gegrepen als oplossing voor dit probleem. In het Vlaams Gewest is tussen 1997 en 2013 een verdubbeling van het gebruik van slaap- en kalmeringsmiddelen waar te nemen. Met een gebruikerspercentage van 13% hoort België bij de grootste gebruikers van slaapmedicatie in Europa. Minister van Volksgezondheid De Block startte in februari 2018 een campagne om dit gebruik te doen dalen. Ze wil meer inzetten op niet-medicamenteuze behandelingen van slapeloosheid. 

Behandeling van slaapproblemen

Momenteel voorzien voornamelijk psychologen slaaptherapie. Zij geven trainingen in cognitieve gedragstherapie of mindfulness en pakken zo het mentale luik van de slaapproblemen aan. Hier komen twee knelpunten naar voor: het aanbod is te gering om aan de omvang van het probleem te voldoen én deelnemers blijven vaak met een hele hoop praktische vragen achter. 

Deze scriptie onderzocht of ergotherapeuten cliënten met slaapproblemen ook kunnen ondersteunen en behandelen. Ergotherapie is een paramedische discipline die het zelfstandig uitvoeren van dagelijkse activiteiten weer mogelijk maakt. Het leren traplopen met krukken aan iemand met een kunstheup of het aanbieden van een gestructureerd stappenplan aan een kind met ADHD om in het regulier onderwijs te kunnen functioneren, zijn hier een goede voorbeelden van. Mensen die slaapproblemen ervaren, kunnen sneller problemen hebben om hun dagelijkse activiteiten en job even goed te blijven uitvoeren.

Personen met een gestoorde slaap functioneren minder goed. Wie vermoeid achter het stuur kruipt, heeft een grotere kans om in een ernstig ongeval betrokken te raken. In de geschiedenis worden zelfs enkele grote rampen, zoals de kernramp van Tsjernobyl en de ontploffing van de Challenger spaceshuttle, toegeschreven aan beslissingsfouten met slaaptekort als veroorzakende factor. Uit onderzoek blijkt eveneens dat werknemers met slaapproblemen minder goed presteren en vaker afwezig zijn. De Verenigde Staten verliezen op deze manier jaarlijks 411 miljard dollar. 

Het bevorderen van het handelen is de kerntaak van een ergotherapeut. Wanneer een ergotherapeut zich richt op de slaapproblemen van een cliënt, kan hij niet alleen de slaap zelf verbeteren, maar eveneens het handelen overdag. Om die reden staan ergotherapeuten de laatste jaren steeds meer open om onderzoek te voeren naar het behandelen van slapeloosheid. Sinds 2009 verschenen de eerste wetenschappelijke publicaties over slaap door ergotherapeuten. In Canada en Amerika, waar de bakermat van de ergotherapie ligt, zijn reeds ergotherapeuten actief die zich richten op slaap. In België en Nederland is dit nog onbekend terrein, daar kan verandering in komen.

Studie bij chronische pijnpatiënten

Aangezien het behandelen van slaapproblemen door ergotherapeuten nog in de kinderschoenen staat, richtte deze scriptie zich op een toegespitste doelgroep die vaak met slaapproblemen kampt: de chronische pijnpatiënten. Patiënten met fibromyalgie, chronische lagerugpijn en chronischevermoeidheidssyndroom werden opgenomen in deze studie omdat zij naast hun pijn vaak slaapproblemen ervaren; of zelfs  insomnia. 

De slaapbegeleiding die aan de deelnemers werd aangeboden, bestond uit drie onderdelen: een zes weken durende slaaptraining, informatieve nieuwsbrieven over slaap en een terugkomdag. De slaaptraining omvatte een interdisciplinaire behandeling waarbij een psycholoog zich richtte op het mentale aspect van slaap en de ergotherapeut zich op de praktische zaken van slaap richtte. Enkele onderwerpen die door de ergotherapeut gegeven werden: de inrichting van de slaapkamer, het belang van een avondritueel, het oefenen van een goede slaaphouding, informeren over goede matrassen en het belang van activiteiten overdag.

Uit deze studie blijkt dat de gemiddelde slaapkwaliteit van de deelnemers een positieve evolutie kende na de slaapbegeleiding. Er kan geconcludeerd worden dat het effectief is om een ergotherapeut als slaapcoach in te schakelen.
