Postoperative fistula and other complications in pancreatic surgery: a retrospective analysis of 255 pancreatic resections

Pancreasfistels: van levensbedreigende complicatie tot beheersbare verwikkeling?

De alvleesklier of pancreas is een orgaan dat veel mensen waarschijnlijk relatief onbekend in de oren klinkt. Toch is pancreaskanker wereldwijd verantwoordelijk voor 200.000 tot 350.000 overlijdens per jaar, wat vergelijkbaar is met het aantal motorrijders die jaarlijks het leven laat bij verkeersongevallen en zo de 4debelangrijkste oorzaak van kankersterfte in de ontwikkelde landen vormt. Waar de afgelopen 30 jaar voor bijna alle kankers grote vooruitgang werd geboekt in de overleving dankzij nieuwe technieken voor diagnose en behandeling, is dit niet het geval voor pancreaskanker. Een chirurgische ingreep blijft tot op vandaag de enige mogelijke kans op genezing. Echter, deze operaties zijn zeer ingrijpend waarbij 30-50% van de patiënten enige vorm van verwikkeling ontwikkelt. Ondanks de globale medische vooruitgang is dit percentage de afgelopen 30 jaar helaas weinig veranderd. Deze complicaties hebben een negatieve impact op de levenskwaliteit van deze patiënten en ook verdere behandelingen kunnen in het gedrang komen, waardoor de overleving negatief kan beïnvloed worden. Het is daarom belangrijk verder onderzoek te doen naar deze verwikkelingen, teneinde deze in de toekomst beter te kunnen inschatten en voorkomen. 

Doel van het onderzoek

Deze thesis, uitgevoerd aan de Universiteit Gent binnen de masteropleiding geneeskunde, had als doel om risicofactoren op te sporen die kunnen gebruikt worden om ‘hoog-risico’ patiënten voor het ontwikkelen van een complicatie te identificeren. Hiertoe werden de medische gegevens van 255 patiënten geanalyseerd, die tussen 2010 en 2015 een operatieve ingreep van de pancreas ondergingen in het Universitair Ziekenhuis Gent. Bijkomend was het doel om de incidentie van complicaties bij deze ingrepen in het UZ Gent te bepalen en deze aantallen te vergelijken met andere centra wereldwijd. In de literatuur wordt immers duidelijk aangetoond dat er een sterke link bestaat tussen het aantal dergelijke operaties die binnen een ziekenhuis worden uitgevoerd en de mortaliteits- en morbiditeitscijfers en dus de uitkomst voor de patiënt. Zogenaamde ‘High-volume’ centra zouden bijgevolg een betere zorg kunnen verschaffen voor de patiënt die een dergelijke complexe chirurgische ingreep moet ondergaan. 

Complicaties bij pancreasoperaties in UZ Gent

De 3 belangrijkste en meest frequente complicaties bij pancreaschirurgie zijn: 1. een vertraagde maaglediging, 2. een bloeding en 3. het ontstaan van een pancreasfistel. Een fistel ontstaat wanneer er een ‘lekkage’ is thv. de nieuwe verbinding tussen de pancreas en de dunne darm, waardoor de verteringssappen zich een vrije weg door het lichaam banen en een ‘holte’ achterlaten met beschadiging van de omliggende weefsels. De thesis heeft zich vooral gericht op fistel vorming gezien deze complicatie de meest catastrofale gevolgen kan hebben en de belangrijkste risicofactor is voor een verlengde hospitalisatieduur en voor het overlijden van de patiënt. In grote lijnen onderscheiden we 2 types van operaties, waarbij in het ene geval de kop van de alvleesklier verwijderd wordt (ook bekend als de ‘Whipple operatie’, rechter deel), en in het andere geval het lichaam en de staart van de alvleesklier verwijderd wordt (linker deel). De keuze van het type operatie is afhankelijk van de positie van de tumor.

In de bestudeerde serie ontwikkelden slechts 4% een klinisch relevante fistel na verwijdering van de pancreaskop. Het UZ Gent scoort daarmee goed gezien dit cijfer laag is in vergelijking met de 10-15% die in andere grote series in de literatuur wordt beschreven. Dit goede resultaat wordt wellicht verklaard door de ervaring bij de chirurgen die deze ingreep op frequente basis uitvoeren, in combinatie met het systematisch gebruik van eenzelfde chirurgische techniek. Dit resultaat ondersteunt dus niet alleen de hypothese dat ervaring van cruciaal belang is bij deze complexe ingrepen, maar laat ook zien dat standaardisatie een belangrijk instrument kan zijn om de resultaten nog verder te verbeteren. 
Bij het verwijderen van de staart van de pancreas ontwikkelt 17,5% van de patiënten een klinisch relevante fistel, hetgeen in lijn ligt met de beschreven resultaten uit andere studies. 

Vertraagde maaglediging na de operatie kwam in het UZ Gent even frequent voor als in andere centra. Het aantal bloedingen na een operatie was wel relatief laag in vergelijking met andere studies, wat een goede operatieve techniek en postoperatieve zorg aangeeft. 

Risicofactoren voor het ontwikkelen van een fistel

Om patiënten met een hoog risico op het ontwikkelen van een fistel te kunnen identificeren, is een goed inzicht in de mogelijke risicofactoren cruciaal. Er werd daarom nagegaan of bepaalde kenmerken zoals lichaamsgewicht, geslacht, type tumor, enzovoort een invloed hadden op het optreden van fistels of andere complicaties. Zo werd geconstateerd dat een zachte textuur van de alvleesklier een belangrijk risico vormt op het ontwikkelen van een fistel, gezien de kans op lekkages groter wordt. Men kan het vergelijken met het hechten van een pudding in vergelijking met het hechten van een stevige biefstuk. Dit is belangrijk omdat nieuwe technieken de mogelijkheid bieden om de textuur van de pancreas voor de operatie te ‘meten’. Dit zou m.a.w. toelaten om voor de ingreep reeds een betere risico inschatting te maken. Voorts werd vastgesteld dat suikerziekte het risico op een fistel vermindert. Dit blijkt logisch gezien de pancreas bij mensen met suikerziekte meestal hard(er) wordt. Tenslotte werd ook gezien dat overgewicht/obesitas het risico op een fistel waarschijnlijk verhoogt. 

In totaal ontwikkelde 25% van de patiënten ten minste 1 van de 3 belangrijkste complicaties. Dit illustreert dat dit type chirurgie nog steeds een erg belastende ingreep is voor de patiënt en dat er op medisch vlak nog belangrijke uitdagingen zijn. De expertise van een centrum in de behandeling van deze complexe ziekte en de vakkundigheid waarmee deze uitdagende ingrepen worden uitgevoerd, blijken belangrijke elementen voor de uiteindelijke uitkomst. Dit belang van expertise wordt geïllustreerd door de goede resultaten die het UZ Gent behaalt naar aantal complicaties. Ook de patiënt zou hiervan op de hoogte moeten zijn wanneer hij/zij behandeld wordt voor deze ernstige ziekte. Complexe ingrepen enkel nog laten plaatsvinden in gespecialiseerde high-volume centra kan een antwoord vormen op de vraag naar een reductie van het aantal complicaties, zodat de patiënt de garantie krijgt op de best mogelijke zorg en levenskwaliteit.
