The prevalence and potential risk factors of suicidal ideation in 8-year-olds in the JOnG! study

Een doodswens bij 8-jarigen

Samenvatting

Deze masterproef houdt één van de eerste studies in die een doodswens specifiek bij jonge kinderen onderzocht. Dit werd nagegaan door de belangrijkste zorgfiguren uit het 6-jarig cohort van de JOnG!-studie bij het tweede meetmoment te vragen of hun 8-jarig kind ooit gezegd had dat hij/zij liever dood wil zijn. Uit het onderzoek bleek dat 12.1% van de zorgfiguren aangeeft dat zijn/haar kind er ooit een doodswens op nahield en dat dit in verband gebracht kan worden met een aantal specifieke factoren.

Zijn 8-jarigen niet te jong om na te denken over zelfdoding?

Onderzoekers hebben lange tijd gedacht dat jonge kinderen geen doodswens konden bezitten omdat hun concept van de dood nog onvoldoende ontwikkeld is. De voorbije jaren werd deze gedachtegang echter ontkracht. Hoewel het klopt dat jonge kinderen over een eenvoudiger concept beschikken, begrijpen ze de fundamentele kenmerken – zoals het onomkeerbaar zijn – ervan al op 4-jarige leeftijd (Barrett & Behne, 2005). Dit betekent dus dat 8-jarigen wel degelijk in staat zijn om een doodswens te ontwikkelen en zo terecht kunnen komen in het suïcidaal proces, bestaande uit respectievelijk: een doodswens, een zelfdodingsplan, een zelfdodingspoging en eigenlijke zelfdoding. Daarnaast lijkt deze laatste fase van het suïcidaal proces een opmars te maken als een van de leidende doodsoorzaken in een steeds jongere populatie. Zo was zelfdoding in 2017 de tweede meest voorkomende doodsoorzaak bij 10 tot 14-jarigen in de Verenigde Staten (Centers for Disease Control and Prevention [CDC], 2019). Daarenboven stelden onderzoekers vast dat eigenlijke zelfdoding op een latere leeftijd hoogstwaarschijnlijker wordt naarmate er op een jongere leeftijd een doodswens aanwezig is (Klimes-Dougan e.a., 1999; Nock e.a., 2008; Velez & Cohen, 1988; Whalen e.a., 2015). Deze zaken benadrukken het belang van onderzoek naar een doodswens op een jonge leeftijd.

Een aanzienlijk aantal risicofactoren die herzien moeten worden

Dankzij het groeiende aantal onderzoekers die zich de laatste decennia bezighouden met het analyseren van het suïcidale proces, werden reeds legio risicofactoren voor een doodswens blootgelegd. Enkele van deze risicofactoren zijn: een psychiatrische stoornis, een conflictueuze relatie met de ouders, ouderlijke zelfdoding en een verwaarlozende opvoedingsstijl (Brent e.a., 2015; Lai & McBride-Chang, 2010; Mayes, Calhoun, Baweja, & Mahr, 2015; Soole & De Leo, 2015). Deze risicofactoren werden vervolgens gebruikt om preventieve en interveniërende maatregelen te ontwikkelen. Echter bracht recent onderzoek een belangrijk leeftijdsspecifiek verschil aan het licht dat significante gevolgen met zich meebrengt voor de wijze waarop onderzoek binnen dit onderwerp tot op heden uitgevoerd wordt. Zij stelden namelijk vast dat de risicofactoren die gelden voor het ontwikkelen van een doodswens tijdens de adolescentie verschillen van deze tijdens de lagere schoolleeftijd (Tan, Xia, & Reece, 2018). De risicofactoren zijn dus met andere woorden leeftijdsafhankelijk. Aangezien de risicofactoren waarop onze huidige preventieve en interveniërende maatregelen gebaseerd zijn, overheersend nagegaan werden binnen steekproeven met variërende leeftijden, betekent dit dat elke risicofactor die binnen dergelijke steekproeven ontdekt werd herzien moet worden. Meer nog, ook de reeds verworpen risicofactoren moeten herbekeken worden.

Door onze steekproef te beperken tot 8-jarigen, hopen wij alvast te kunnen bijdragen aan deze herziening. Om dit te kunnen verwezenlijken, maakten we gebruik van vragenlijsten die ingevuld werden in het kader van de JOnG!-studie. Deze studie stelt een onderzoek voor naar de ontwikkeling, opvoeding, gezondheid en gedrag bij kinderen en hun gezin. Aan de hand van de antwoorden die zorgfiguren over hun 8-jarig kind gaven, konden verschillende potentiele risicofactoren blootgelegd worden.

Deze factoren zijn mogelijks alarmerend

Uit ons onderzoek kwam naar voren dat 12.1% van de zorgfiguren melden dat hun kind ooit blijk gaf van een doodswens en dit gemiddeld op 7 à 8-jarige leeftijd. Het jongste kind waarbij suïcidale gedachten opgemerkt werden, was amper 3 jaar oud. De belangrijkste zorgfiguren gaven aan dat een doodswens vooral tot uiting kwam wanneer hun kind onder de invloed was van sterke emoties, bij ernstige familiale conflicten zoals een scheiding of een overlijden, bij conflicten met vrienden en problemen op school zoals gepest worden. Opmerkelijk was dat de meeste zorgfiguren op het moment van deze uitingen maar ‘een beetje’ ongerust waren. Bij de kinderen waarbij een doodswens gerapporteerd werd, kwamen de hieronder beschreven factoren significant vaker voor.

Ten eerste leek een doodswens zich vaker te ontwikkelen in de aanwezigheid van een of meerdere psychiatrische diagnose(s) en meer specifiek bij ADHD en een autismespectrumstoornis. Deze eerste bevinding toonde al meteen het leeftijdsspecifieke karakter van de risicofactoren aan. Bij onderzoeken die tevens oudere kinderen includeerden in hun steekproef, werden namelijk ook angst- en stemmingsstoornissen in verband gebracht met een doodswens. Ten tweede werd een doodswens in verband gebracht met impulsief en agressief gedrag. Dit is in lijn met eerder onderzoek en correspondeert tevens met de hierboven vernoemde aanwezigheid van ADHD, aangezien impulsiviteit bij deze ontwikkelingsstoornis een belangrijke component voorstelt. Ten derde bleek het meemaken van meerdere (stressvolle) gebeurtenissen tegelijkertijd ook gerelateerd te zijn aan een doodswens. Verder werd één specifieke gebeurtenis in verband gebracht met een doodswens, namelijk een significante toename van het gezinsinkomen. Dit is opmerkelijk aangezien deze gebeurtenis nog nooit eerder binnen dit onderzoeksveld onderschreven werd. Tot slot werd er binnen dit onderzoek gekeken naar het in verband staan van een doodswens met bepaalde opvoedingsstijlen. Een doodswens kwam minder voor indien de zorgfiguren meer regels en positief ouderlijk gedrag toepasten, minder (fysiek) hard straften en minder probeerden binnen te dringen in de emotionele en psychologische ontwikkeling van hun kind.

Vertaling naar de praktijk

Deze studie houdt één van de eerste onderzoeken in dat een doodswens specifiek bij jonge kinderen onderzocht. Onderzoek hiernaar is nochtans zinvol aangezien een recent studie uitwees dat de risicofactoren, waarop onze huidige preventieve en interveniërende maatregelen gebaseerd zijn, leeftijdsafhankelijk zijn. Verder kunnen de onderzoeksresultaten een aanzet geven voor een ontwikkeling van een screeningsinstrument voor vroegdetectie van suïcidaliteit in de basisschool. Tot slot informeren onze bevindingen hulpverleners om waakzaam te zijn voor een doodswens op jonge leeftijd naargelang de aanwezigheid van een (of meerdere) psychiatrische diagnose(s) zoals ADHD en autismespectrumstoornis, de mate van impulsief en agressief gedrag, meegemaakte (stressvolle) levensgebeurtenissen en de opvoedingsstijl die toegepast wordt door de zorgfiguren.
