To what extent does 3D static alignment represent the dynamic function of the spine during gait in Adult Spinal Deformity: a pilot study

In welke mate is het 3D statische alignement representatief voor de dynamische functie van de wervelkolom in patiënten met adulte spinale deformiteit?

Adulte spinale deformiteit (ASD) houdt een breed spectrum van wervelkolomaandoeningen in. Het heeft een grote impact op het functioneren van deze patiënten, specifiek op hun balans. Uit eerder onderzoek blijkt dat ASD patiënten compenseren voor hun aandoening door op een andere manier te staan en wandelen dan controlesubjecten. Er blijkt ook een discrepantie te zijn tussen de houding in stand en deze tijdens wandelen.

De evaluatie van deze patiënten gebeurt tot op heden meestal op basis van radiografische metingen. Het probleem hierbij is dat deze metingen in een tweedimensionaal (2D) vlak gebeuren, terwijl ons lichaam uiteraard in het driedimensionaal (3D) vlak beweegt. Of er al dan niet een operatie - en welk type - gepland wordt, is tegenwoordig voornamelijk gebaseerd op deze radiografische metingen.

Wij geloven dat het belangrijk is om na te gaan of deze methode accuraat is, aangezien heel wat operaties falen of revisie nodig hebben. Om die reden gingen wij na of de radiografische metingen betrouwbaar zijn en of er alternatieven beschikbaar zijn.

We voerden een vergelijkende studie uit tussen 2D radiografische, 3D statische en 3D dynamische metingen bij 10 ASD patiënten en 1 controlesubject. Voor de 3D metingen maakten we gebruik van een subject-specifiek model. Dit betekent dat de wervelkolom en het bekken als 3D modellen werden gevisualiseerd, waardoor geïndividualiseerde calculaties konden gemaakt worden. In een ganglabo werden de patiënten geïnstrueerd om te wandelen, waaruit we de 3D dynamische beelden en berekeningen hebben gehaald, en om stil te staan, wat de 3D statische waarden opleverde.

Aangezien de groep heel klein en vrij heterogeen is, konden wij geen statistiek toepassen op de verkregen data. Toch hadden we voldoende informatie om belangrijke conclusies te kunnen trekken.

Enerzijds besluiten we dat radiografie alleen onvoldoende informatie biedt over het dynamische gedrag van de wervelkolom bij ASD patiënten. De radiografische metingen zijn sterk afhankelijk van de onderzoeker, kleine inschattingsfouten leveren al snel grote verschillen op in de gemeten parameters. We merkten dat de radiografische waarden vaak sterk afweken van de dynamische en onderzochten daarom ook de overeenkomsten tussen de 3D statische en 3D dynamische parameters. De verschillen tussen deze laatste lagen heel wat lager, 3D statische evaluatie lijkt dus een betere voorspeller te zijn van het dynamische gedrag.

Anderzijds gingen we op zoek naar de verschillen tussen de statische en dynamische parameters, die weergeven in welke mate ASD patiënten zich anders gedragen in stand dan tijdens wandelen. Uit deze vergelijking kunnen we enkele zaken besluiten: ASD patiënten wandelen -net zoals gezonde personen- met de romp meer voorovergebogen dan ze staan. Tijdens stand maken ze gebruik van een achterwaartse bekkenkanteling, die ze verliezen bij het wandelen.

Dit onderzoek geeft een eerste aanzet naar het evalueren van ASD subjecten op basis van dynamische en gepersonaliseerde informatie. Meer onderzoek is nodig om dit verder te exploreren.
