De mobile stroke unit : een rijdend ziekenhuis voor CVA patiënten

Cerebrovasculair accident

Een cerebrovasculair accident (CVA), ook wel een beroerte genoemd, is een aandoening waarbij de bloedtoevoer naar een deel van de hersenen verstoord is. Hersencellen kunnen niet lang zonder adequate bloedtoevoer overleven, en sterven tijdens een CVA dus massaal af. Hierdoor loopt de patiënt permanente hersenschade op, wat drastische gevolgen kan hebben. Wereldwijd overlijdt ongeveer een derde van de CVA patiënten en blijft nog eens een derde permanent zorgbehoevend.  Een CVA is daarmee de derde meest voorkomende doodsoorzaak.

Het is bij een CVA van cruciaal belang om de bloedtoevoer naar de hersenen zo snel mogelijk te herstellen. Zo sterven er namelijk minder hersencellen af en verhoogt de kans op goede uitkomst. Patiënten die binnen de 90 minuten de geschikte behandeling krijgen, hebben dubbel zoveel kans om geen blijvende gevolgen van de beroerte over te houden als patiënten die pas na drie uur behandeld worden. Vandaar de bekende uitspraak ‘Time is brain’.

Ongeveer 80% van de beroertes wordt veroorzaakt door een klonter in de hersenslagaderen. Een gebruikelijke behandeling daarvoor is het toedienen van een klonteroplossend middel. In sommige gevallen volstaat dit niet en moet de klonter operationeel verwijderd worden.

Niet alle ziekenhuizen bieden deze behandelingen aan. Daarom moeten vaak tijdrovende interhospitaaltransfers ondernomen worden, wat nefaste gevolgen kan hebben voor de patiënt.

 

Mobile stroke unit

Dit probleem kan verholpen worden door de mobile stroke unit (MSU). Deze gespecialiseerde ziekenwagen is volledig uitgerust om de CVA patiënt nog voor aankomst in het ziekenhuis te diagnosticeren. De patiënt kan dan onmiddellijk naar het ziekenhuis gebracht worden dat de juiste behandeling aanbiedt, zodat nodeloze interhospitaaltransfers vermeden worden. Daarenboven kan in de MSU ook de behandeling met het klonteroplossend middel al bij de patiënt thuis starten. Dit levert de patiënt waardevolle tijdswinsten op. UZ Leuven plant om een MSU aan te schaffen.  Daarom verrichten zij momenteel onderzoek naar wat nu net de impact zou zijn van een dergelijke gespecialiseerde ziekenwagen voor patiënten met een beroerte in de omgeving van Leuven.

De ervaring leert namelijk dat de manier waarop de MSU geïmplementeerd wordt een grote invloed kan hebben op hoeveel baat de patiënten er werkelijk bij hebben. In deze thesis is de zorg voor CVA patiënten in de ruime omgeving van Leuven gesimuleerd om in te schatten wat de voornaamste invloedsfactoren zijn en hoe deze verbeterd kunnen worden zodat de patiënten zoveel mogelijk voordeel halen uit deze MSU.  Tijdens dit onderzoek is aan het licht gekomen dat de drieminutenregel en de performantie van de 112 de meest ingrijpende invloed hebben op de effectiviteit van de MSU.

 

De drieminutenregel

Wanneer een patiënt naar de nooddiensten belt voor een medisch probleem, kunnen drie voertuigen uitgestuurd worden: een ziekenwagen, een PIT of een MUG met ziekenwagen assistentie. Welk voertuig wordt uitgestuurd hangt af van de urgentie van het noodgeval. In de meeste situaties volstaat een gewone ziekenwagen. Is de patiënt in onmiddellijk levensgevaar, dan wordt een MUG uitgestuurd. Een PIT verkrijgt de voorkeur bij ernstige situaties, maar waar geen acuut levensgevaar dreigt.
De PIT verpersoonlijkt dus de ‘gulden middenweg’ van de noodvoertuigen. De invoer van deze voertuigen is nog maar recent gebeurd en er zijn er nog niet heel veel van actief. Daarom moet de PIT soms erg lang rijden om de patiënt te bereiken. Om deze lange rijtijden te beperken is de drieminutenregel in het leven geroepen. Deze regel houdt in dat wanneer er een ziekenwagen meer dan drie minuten dichter bij de patiënt is, deze voorrang krijgt op de PIT.

De MSU is ook onderhevig aan deze regel. En dat heeft grote gevolgen! Aangezien ziekenwagens zo wijd verspreid zijn over Vlaanderen, zal er bijna altijd wel ergens een ziekenwagen meer dan drie minuten dichter bij de patiënt zijn dan de MSU. De MSU zal maar zeer zelden kunnen uitrijden en zijn potentieel komt niet tot uiting. Een uitzondering maken op de drieminutenregel voor de MSU is dus een eerste essentiële voorwaarde om de MSU tot een succesverhaal te maken.image-20200706005740-2

De 112

De noodlijn 112 vormt de verbinding tussen patiënten en medische bijstand. Voor een gespecialiseerd voertuig zoals de MSU spelen zij een cruciale rol. Om efficiënt gebruik te kunnen maken van de MSU is het namelijk nodig dat de noodlijn oproepen voor CVA patiënten correct kan uitfilteren.

Het blijkt in de praktijk niet altijd eenvoudig om CVA patiënten na een kort telefoongesprek als dusdanig te herkennen. Slechts 30 tot 40% van alle beroertepatiënten wordt effectief door de noodlijn geïdentificeerd.

In Berlijn is een vragenlijst ontworpen om de noodlijn te helpen bij het uitfilteren van CVA patiënten. Na implementatie steeg de herkenningsgraad naar 50 tot 60%. Een dergelijke verbeterde performantie van de noodlijn stelt de MSU in staat om extra CVA patiënten op te vangen en ook deze patiënten verbeterde zorg te bieden.

Illusie of werkelijkheid?

De gemiddelde tijd tot toediening van het klonteroplossend middel is voor patiënten in de MSU 35 tot 45 minuten korter dan normaal. Deze indrukwekkende tijdswinst doet vele neurologen dromen over een ‘mobile stroke unit revolutie’.

Hoeveel CVA patiënten ook werkelijk door de MSU worden opgevangen is echter sterk afhankelijk van bijvoorbeeld de drieminutenregel en de kwaliteit van de informatie van de 112, maar ook van andere factoren zoals de organisatie van de bemanning en de werkuren van de MSU.
In gunstige omstandigheden, wordt tot 35% van alle CVA patiënten opgevangen door de MSU. Zijn de omstandigheden niet goed aangepast aan de vereisten voor de MSU, is dit slechts 3%. Het voordeel van de MSU wordt dan bijna volledig teniet gedaan.

De les is duidelijk: de MSU heeft het potentieel om de zorg voor CVA patiënten aanzienlijk te verbeteren, maar voldoende aandacht voor cruciale invloedsfactoren is noodzakelijk. Bij ondoordachte implementatie wordt deze ingenieuze ziekenwagen namelijk niet efficiënt gebruikt.
