The Psychological Needs of the Fourth Age: The Role of Ego-Integrity and Contextual Need Support in Nursing Homes

De psychologische behoeftehuishouding van rusthuisbewoners: de donkere én heldere kant

Stel: ik vraag u het leven van een rusthuisbewoner te beschrijven. De kans is reëel dat u een gelaten rusthuisbewoner beschrijft in een muffe kamer die melancholiek terugblikt op het verleden. De beruchte Pano-reportage in oktober 2017 over de wantoestanden in commerciële woonzorgcentra kwam die beeldvorming niet ten goede. Ook tijdens de COVID-19 crisis worden bewoners van een woonzorgcentrum in de media vaak opgevoerd als slachtoffers overgeleverd aan de situatie. Een weinig hoopgevende boodschap, maar misschien kan deze scriptie daar verandering in brengen.

Prangende vragen

Rusthuisbewoners staan voor allerlei uitdagingen, zoals fysieke achteruitgang en het zich aanpassen aan de nieuwe leefomgeving. Rusthuisbewoners kunnen veerkrachtig omgaan met veranderende levensomstandigheden en op die manier een hoog niveau van welbevinden behouden. De confrontatie met fysieke achteruitgang en sociale verliezen maakt rusthuisbewoners wel meer kwetsbaar voor de ontwikkeling van psychosociale problemen, waaronder depressie. Bovendien toont wetenschappelijk onderzoek aan dat er veel variatie is tussen rusthuisbewoners in termen van psychologisch welbevinden. Deze verschillen tussen mensen stellen het stereotype beeld van de gelaten rusthuisbewoner, die soms eenzijdig wordt benaderd als een slachtoffer, in vraag. Hoe verklaren we die variatie in psychologisch welbevinden? Wat is de dynamiek tussen het zorgklimaat in een woonzorgcentrum en het psychologisch welbevinden van rusthuisbewoners? Tot slot, welke invloed heeft een rusthuisbewoner zelf op zijn/haar relatie met het zorgteam?

Bovenstaande onderzoeksvragen brachten ons bij de Zelf-Determinatietheorie van Richard Ryan en Edward Deci (2017). Naast lichamelijke basisbehoeften, zoals honger, dorst en slaap, heeft iedereen volgens de Zelf-Determinatietheorie drie fundamentele psychologische basisbehoeften. Mensen hebben nood aan vrije wil en keuze (autonomie), het gevoel dat ze activiteiten tot een goed einde kunnen brengen (competentie) en een warme band met anderen (relationele verbondenheid). Hoe meer de omgeving deze behoeften ondersteunt, hoe beter mensen zich in hun vel voelen en veerkrachtig kunnen omgaan met veranderde levensomstandigheden.  

Naast een theorie gingen we ook op zoek naar deelnemers. Een selectieprocedure resulteerde in een steekproef van 47 rusthuisbewoners (66% vrouwen). Met een gemiddelde leeftijd van 88 jaar is het bovendien één van de oudste steekproeven ooit bestudeerd binnen de Zelf-Determinatie theorie. Om tot betrouwbare antwoorden te komen, sprak de onderzoeker intensief met elke deelnemer en begeleidde hen in het invullen van de vragenlijsten. Alle vragenlijsten zijn afgenomen net voor de eerste lockdown in maart 2020 werd ingeroepen.

De donkere en heldere kant van het verhaal

De meerderheid van de bevraagde rusthuisbewoners blikte positief terug op het verleden en was doorgaans tevreden met de huidige levensomstandigheden. Een vierde van de bewoners vertoonde daarentegen wel signalen van een mogelijke depressie. Gevoelens van eenzaamheid verschilden dan weer sterk onder rusthuisbewoners.

Een potentiële verklaring voor deze variatie in het psychologisch functioneren van rusthuisbewoners vinden we terug bij de psychologische basisbehoeften. Rusthuisbewoners die zich bekwaam voelen en vertrouwen hebben in hun kunnen (competentie), zijn meer tevreden met de huidige levensomstandigheden en rapporteren minder depressieve gevoelens. Die bevinding geldt eveneens voor rusthuisbewoners die een gevoel van keuze hebben en zich niet gedwongen voelen om te moeten deelnemen aan activiteiten waar ze zelf niet voor zouden kiezen (autonomie). Tot slot, een meer voor de hand liggende bevinding: bewoners van het woonzorgcentrum die zich verbonden voelen met mensen die voor hen belangrijk zijn (relationele verbondenheid), rapporteren minder gevoelens van eenzaamheid. De psychologische basisbehoeften kunnen dus een mogelijke verklaring bieden waarom sommige rusthuisbewoners erin slagen een bepaald niveau van welbevinden te behouden, terwijl anderen meer kwetsbaar zijn.

Goede zorgrelatie neemt eenzaamheid niet weg

Aangezien rusthuisbewoners bij uitstek afhankelijk zijn van het zorgteam, is het belangrijk om ook de vraag te stellen in hoeverre het zorgteam invloed heeft op die psychologische basisbehoeften. Het zorgteam kan een verschillende zorgstijl hanteren die de psychologische basisbehoeften ondersteunt dan wel ondermijnt. Een behoefte-ondersteunende zorgstijl staat voor het aanbieden van zinvolle en boeiende activiteiten, empathisch luisteren en uitnodigend taalgebruik. Een behoefte-ondermijnende zorgstijl betekent daarentegen dat het zorgpersoneel bepaalde taken overneemt die bewoners zelf kunnen en willen uitvoeren of minder vriendelijk is wanneer bewoners een andere mening hebben. Een opvallende bevinding, in het licht van de soms negatieve berichtgeving over de zorg in woonzorgcentra, is dat rusthuisbewoners gemiddeld het zorgklimaat als eerder behoefte-ondersteunend ervoeren. De resultaten toonden verder, zoals theoretisch verwacht, positieve verbanden aan tussen een behoefte-ondersteunende zorgstijl, de psychologische basisbehoeften autonomie en competentie, en het welbevinden van rusthuisbewoners. Een behoefte-ondersteunende zorgstijl vertoonde echter geen verband met de basisbehoefte aan relationele verbondenheid. Een mogelijke verklaring hiervoor is dat rusthuisbewoners een gevoel van warmte en verbondenheid niet zozeer halen uit interacties met het zorgpersoneel, maar wel uit interacties met medebewoners, familieleden, of zelfs herinneringen uit het verleden.

Een laatste, maar zeker niet onbelangrijke toevoeging aan deze scriptie was dan ook de vraag op welke manier rusthuisbewoners invloed hebben op de zorgstijl van het personeel in een woonzorgcentrum. Doordat rusthuisbewoners vaak afgebeeld worden als passief overgeleverd aan de situatie, vergeten we dat ook zij actief bijdragen aan interacties met het zorgpersoneel. Zorgen voor een kwetsbare bewoner die met wanhoop terugblikt op het verleden is een andere opdracht dan zorgen voor een veerkrachtig iemand die dankbaar in het leven staat en negatieve levensgebeurtenissen een plaats kan geven. Op sommige vlakken bleken kwetsbare ouderen inderdaad meer vatbaar te zijn voor een weinig behoefte-ondersteunend zorgklimaat. Die bevinding geeft ook aanleiding tot andere onderzoeksvragen, die specifiek de actieve rol van de bewoners in hun interacties met de omgeving centraal stellen. Hoe gaan kwetsbare rusthuisbewoners, in vergelijking met veerkrachtige bewoners, om met een bepaalde zorgstijl? Interpreteren bewoners bepaalde zorginteracties op een verschillende manier? Of ontlokken ze bij de zorgverleners een andere interactiestijl?

Een uitgestoken hand naar ouderen

Deze scriptie onderstreept het belang van een behoefte-ondersteunend zorgklimaat voor het welbevinden van rusthuisbewoners en in het bijzonder voor meer kwetsbare ouderen. De bevindingen stellen ook de stereotype beeldvorming over ouderen in woonzorgcentra in vraag, en bieden handvaten om als zorgteam in een woonzorgcentrum het psychologisch welbevinden van bewoners te ondersteunen. Gegeven de wonderbaarlijke veerkracht die veel van deze ouderen vertonen in moeilijke omstandigheden, waaronder ook de COVID-19 crisis, verdienen ze ten volle zorg van hoge kwaliteit.
