The anti-inflammatory effect of mesenchymal stem cell-derived extracellular vesicles in Niemann-Pick disease type C1 pathology

Eerste stappen in zoektocht naar behandeling tegen NPC1 gezet?

 

Eén op 120 000 tot 150 000 levend geboren baby’s in West-Europa krijgt de diagnose van de ziekte van Niemann-Pick type C1 (NPC1). De ziekte heeft  een breed klinisch spectrum, gaande van leverontsteking tot dementie. In het laboratorium van professor Roosmarijn Vandenbroucke in VIB-UGent Center For Inflammation Research, zetten onderzoekers hoopgevende stappen naar de ontwikkeling van een nieuwe behandelingsstrategie, aan de hand van onderzoek op proefdiermuizen.

 

Wat is NPC1?

De ziekte van Niemann-Pick type C1 (NPC1) is een erfelijke stofwisselingsziekte die de hersenen, maar ook andere organen zoals de lever en de longen, kan aantasten. De symptomen kunnen sterk verschillen van patiënt tot patiënt. Deze omvatten een vergrote lever en/of milt, ontstekingen van de lever, milt, longen en hersenen, en neurologische symptomen zoals spraakstoornissen, dementie en stuiptrekkingen. De oorzaak van de ziekte is een mutatie in het Npc1-gen, resulterend in een niet-functioneel eiwit. Als gevolg hiervan stapelt cholesterol zich op in de lysosomen, de afvalverwerkers van de cellen, waardoor de celfunctie sterk verstoord kan raken.

Op dit moment zijn enkel symptomatische behandelingen, bijvoorbeeld medicatie die stuiptrekkingen onderdrukken, beschikbaar tegen NPC1, zonder dat de oorzaak van de ziekte hiermee wordt aangepakt. Dit project onderzoekt een nieuwe behandelingstechniek die gebaseerd is op mesenchymale stamcel-afgeleide extracellulaire vesikels (MSC-EVs).

 

MSC-EV? Wat ben je ermee?

Mesenchymale stamcellen zijn belangrijke cellen voor het lichaam omdat deze jonge cellen zichzelf kunnen ontwikkelen tot ondersteunende cellen in bijvoorbeeld beenderen. Een ander kenmerk van mesenchymale stamcellen is dat ze ontstekingsremmende en hernieuwende eigenschappen bezitten.

MSC-EV staat voor mesenchymale stamcel-afgeleide extracellulaire vesikels. Zoals de naam doet uitschijnen, zijn dit vesikels – anders gezegd een soort transportpakjes – die door mesenchymale stamcellen worden uitgescheiden om een boodschap over te brengen naar een andere plaats in het lichaam. Een basiseigenschap van EVs is dat deze vaak gelijkaardige eigenschappen bezitten als de cellen waarvan ze afkomstig zijn. MSC-EVs zouden dus ook ontstekingsremmende en hernieuwende eigenschappen bezitten. Andere onderzoeken toonden ook al aan dat MSC-EVs ontstekingsremmende effecten hebben in andere ziektes, zoals multipele sclerose. In dit onderzoek besloten we te werken met extracellulaire vesikels omdat deze ook makkelijk de hersenen kunnen bereiken.

 

Gewichtstoename na behandeling

Terug naar het laboratorium van professor Vandenbroucke, waar we MSC-EVs testten als nieuwe behandelingsstrategie voor NPC1. Hiervoor werden op meermaals MSC-EVs toegediend aan muizen met een niet-functioneel NPC1-eiwit (Npc1-/--muizen). Vervolgens bekeken we het effect van deze behandeling op zowel typische neurologische symptomen, als op symptomen op andere plaatsen in het lichaam. De resultaten werden vergeleken met de symptomen in Npc1-/--muizen die een placebobehandeling kregen. Een laboratorium van het Universitair Ziekenhuis van Essen werkte mee aan dit onderzoek door de MSC-EVs te voorzien die men isoleerde uit een vrijwillige donor.

Patiënten met NPC1 vertonen doorgaans een vergrote milt en ook hun groeicurve en gewichtstoename kunnen afwijken van het gemiddelde. Deze kenmerken kon men ook zien in het gebruikte muismodel voor dit onderzoek. Na behandeling met MSC-EVs stelden we vast dat deze therapie een positief effect had op zowel miltgrootte als gewichtstoename, terwijl de placebobehandeling geen effect had (Figuur 1).

Behandeling vermindert ontsteking in hersenen en organen

In een volgend onderzoek gingen we de aanwezigheid van typische onstekingsmerkers na in verschillende organen, namelijk de hersenen, longen, lever en milt. Deze analyses toonden aan dat Npc1-/--muizen die een MSC-EV-behandeling kregen, algemeen een lager niveau van ontsteking vertoonden in vergelijking met zieke muizen die de placebobehandeling kregen. Bovendien toonde dit experiment ook aan dat het niveau van ontsteking in de met MSC-EV behandelde muizen zich herstelde tot een vergelijkbaar niveau als bij gezonde muizen.

 

Minder hersenschade

NPC1 wordt ook gekenmerkt door een stijging van specifieke hersencellen die betrokken zijn bij het proberen te bestrijden van hersenschade en -ontsteking. Deze cellen, astrocyten en microglia genaamd, werden gevisualiseerd met behulp van microscopie in de hersenen van de gezonde muizen en met placebo of MSC-EV behandelde zieke muizen. Astrocyten zijn cellen die onder andere deel uitmaken van de bloed-hersenbarrière, maar ook beschadigde hersencellen herstellen. Microglia zijn immuuncellen in de hersenen die schadelijke stoffen proberen te neutraliseren.

Opnieuw werden veelbelovende resultaten waargenomen. Er waren minder astrocyten waarneembaar bij zieke muizen na behandeling met MSC-EVs in vergelijking met zieke muizen die behandeld werden met een placebo (Figuur 2). Eenzelfde effect werd waargenomen wanneer we keken naar microglia. Beide celtypes worden geactiveerd als hersenschade optreedt, wat dus minder het geval blijkt te zijn na toediening van MSC-EVs.

Baanbrekend werk verzet?

De resultaten in onze studie openen nieuwe perspectieven voor de ontwikkeling van een behandelingsstrategie tegen NPC1. Of met dit project baanbrekend werk werd verricht, zal nog moeten blijken uit extra onderzoek op proefdieren. Het grote voordeel aan MSC-EVs is dat deze in tegenstelling tot behandelingen met cellen minder kans hebben op tumorvorming en daarnaast ook minder vaak door de patiënt worden afgestoten. Indien hieruit een geneesmiddel kan ontwikkeld worden, moet dit ook eerst nog op gezonde mensen worden getest. Het zou dus nog jaren kunnen duren voordat het eventuele geneesmiddel op de markt komt. Wat wel vaststaat, is dat de onderzoekers waarmee ik samenwerkte aan dit uitdagend project enthousiast zijn over de verkregen resultaten. ‘Dit onderzoek toont ons dat een volledig vernieuwende behandelingsstrategie ons in staat stelt om uiteenlopende symptomen typerend voor NPC1 te verminderen in muizen. De resultaten in onze studie openen heel wat perspectieven voor toepassing van de therapie bij de mens.’, klinkt het bij Lien Van Hoecke, postdoctorale medewerker in dit onderzoek.
